血清RBP4、Apelin水平与糖尿病视网膜病变的相关性

摘　　要：目的探讨血清视黄醇结合蛋白4(RBP4)、Apelin水平与糖尿病视网膜病变(DR)的相关性。方法选取2017年10月至2019年4月接受治疗的162例2型糖尿病(T2DM)并发DR患者,依据DR分期标准分为增生期糖尿病性视网膜病变(PDR)组90例和非增生期糖尿病性视网膜病变(NPDR)组72例,另选取同期接受治疗的单纯T2DM患者106例为T2DM组。3组患者均于入院时检测血糖、血脂指标及血清RBP4、Apelin水平,比较3组患者上述指标的差异,并分析T2DM患者并发DR的危险因素。结果3组患者糖尿病病程、FPG、HbA1c、FINS、HOMA-IR、TC、HDL-C比较,差异均有统计学意义(P<0.05);PDR组、NPDR组血清RBP4、Apelin水平均明显高于T2DM组,且PDR组高于NPDR组,组间比较差异均有统计学意义(P<0.05);多因素Logistic回归分析结果显示,T2DM病程、HbA1c、HOMA-IR、HDL-C、RBP4、Apelin均是T2DM患者并发DR的危险因素(P<0.05)。结论DR患者血清RBP4、Apelin水平显著升高,RBP4、Apelin的高水平表达与DR的发生、病情进展密切相关。Objective To investigate the correlation between the serum levels of retinol binding protein 4(RBP4),Apelin and diabetic retinopathy(DR).Methods A total of 162 patients with type 2 diabetes(T2DM)complicated by DR who were treated in our hospital from October 2017 to April 2019 were enrolled in the study,who were divided into proliferative diabetic retinopathy(PDR)group and non-proliferative diabetic retinopathy(NPDR)group according to the DR staging criteria.At the same time,the other 106 patients with simple T2DM were enrolled as T2DM group.The blood glucose,lipids and serum levels of RBP4,Apelin on admission were detected and compared among the three groups.Moreover the risk factors for incidence of DR in patients with T2DM were analyzed.Results There were significant differences in the course of diabetes,FPG,HbA1c,FINS,HOMA-IR,TC,and HDL-C among the three groups(P<0.05).The serum levels of RBP4,Apelin in PDR group and NPDR group were significantly higher than those in T2DM group(P<0.05),moreover,which in PDR group were significantly higher than those in NPDR group(P<0.05).Multivariate logistic regression analysis showed that the course of diabetes,HbA1c,HOMA-IR,HDL-C,RBP4,Apelin were the risk factor for incidence of DR in patients with T2DM(P<0.05).Conclusion The serum levels of RBP4 and Apelin in patients with DR are significantly increased,and the increase of the expression levels of RBP4 and Apelin is closely related to the occurrence of DR and disease progression.
