听觉剥夺时间对语后聋患者耳蜗术后听觉效果影响  

Effects of Duration of Auditory Deprivation on Speech Rehabilitation after Cochlear Implantation in Adult with Postlingual Deafness

摘　　要：目的探讨听觉剥夺时间对重度或极重度成人语后聋患者人工耳蜗植入术后听觉言语康复效果的影响。方法收集行人工耳蜗植入术的36例成人语后聋患者的临床资料,按听觉剥夺时间分为A、B两组,A组<10年、10年≤B组≤20年,并分别记录两组患者术后3个月、6个月听觉行为分级标准(categories of auditory performance,CAP)及有意义听觉整合量表(meaningful auditory integration scale,MAIS)的得分,分析比较听觉言语康复效果。结果A组术后3个月、6个月CAP得分分别为4(0,7)和6(1,9),二者之间的差异有统计学意义(P<0.05),A组患者CI术后3个月、6个月MAIS得分分别为23(8,32)和230(10,40),二者之间的差异有统计学意义(P<0.05);B组术后3个月、6个月CAP得分分别为3.46±1.85和5.00±2.38,二者之间的差异有统计学意义(P<0.05),B组患者CI术后3个月、6个月MAIS得分分别为16.38±6.12和25.00±8.81,二者之间的差异有统计学意义(P<0.05);术后3个月A、B两组CAP得分分别为4(0,7)和3.46±1.85,二者之间的差异无统计学意义(P>0.05);CI术后3个月A、B两组MIS得分分别为23(8,32)和16.38±6.12,二者之间的差异无统计学意义(P>0.05);术后6个月A、B两组CAP得分分别为6(1,9)和5.00±2.38,二者之间的差异无统计学意义(P>0.05);CI术后6个月A、B两组MAIS得分分别为30(10,40)和25.00±8.81,二者之间的差异无统计学意义(P>0.05)。结论人工耳蜗植入可有效改善成人语后聋患者听觉能力,积极的进行术后康复对于患者言语能力提高有明显效果;听觉剥夺时间不同的患者在本研究中术后听觉言语康复效果无明显区别,听觉剥夺时间长的患者在CI术后依然可以获得效果。Objective To investigate the effect of time of auditory deprivation on post-cochlear implant auditory speech rehabilitation outcomes in patients with severe or profound postlingual deafness.Method Based on their auditory deprivation time,36 adult patients with postlingual deafness were divided into group A(<10 years)and group B(10 to 20 years).The categories of auditory performance(CAP)and meaningful auditory integration scale(MAIS)scores were used to measure auditory speech rehabilitation outcomes at postoperative 3 and 6 months and compared between the two groups.Results Both CAP and MAIS scores improved at 6 months as compared to 3 months(P<0.05):CAP=6(1,9)vs 4(0,7)and MAIS=30(10,40)vs 23(8,32)for group A;and CAP=5(0,9)vs 4(0,6)and MAIS=25.00±8.81 vs 16.38±6.12 for group B.CAP and MAIS scores were not significantly different between the two groups(P>0.05)at either 3 or 6 months.Conclusion Cochlear implants can effectively improve hearing ability in adult patients with postlingual deafness with positive postoperative rehabilitation outcomes and improvement of speech ability.In this study,no significant difference in speech rehabilitation outcomes were seen between patients with different duration of auditory deprivation,suggesting that patients with long duration of auditory deprivation can still benefit from CI.
