摘 要:内耳是一个复杂、结构精细、受严格调控的感觉器官,易受到内在和外在因素的干扰,导致暂时性或永久性的听力损伤[1]。主要原因有噪声暴露、耳毒性药物、老龄化、自身免疫性疾病等[2-3],由其引起的听力减退、耳闷胀感、眩晕等症状极大地降低了患者的生活质量。目前临床上使用助听器、人工听觉植入等辅助设备来部分或完全恢复听力,但并没有恢复潜在的病理变化[4]。As isolated anatomical position, limited labyrinthine artery supply, and blood-labyrinth barrier hampers systemic drug delivery to the inner ear. The efficient concentration of drug treatment is unsatisfactory and there’s possible side effects after systemic administration. Intratympanic injection of drug can bypass the blood-labyrinth and permeated to the hair cells or synaptic area via the round-or oval window of the cochlea. Efficacy and safety of pharmacotherapy has become increasingly relied on the inner ear delivery carrier system. The goal of this review focus on the anatomical barrier that need to be overcome in the intratympanic applications, the improvement of drug retention and specific targets, and the safety of novel drug carriers, these emerging strategies of local drug delivery promise novel and better guidance for the clinical application.
