基于综合护理干预对局麻术下行腹股沟疝手术患者围手术期指标影响及心理状态改善研究

摘　　要：目的探讨基于综合护理干预对局麻术下行腹股沟疝手术患者围手术期指标影响及心理状态改善效果。方法选取2018年1月至2019年7月,首都医科大学附属北京朝阳医院120例局麻术下行腹股沟疝手术患者,随机分为观察组与对照组,每组60例。对照组给予常规护理干预,观察组在对照组的基础上给予综合护理干预,比较2组患者心理应激状态、术中配合、焦虑抑郁情绪及心境状态。结果观察组术前30 min、入室30 min后血压、心率水平低于对照组(P<0.05);观察组术中疼痛、恐惧评分低于对照组,术中配合度评分高于对照组(P<0.05);观察组干预后SAS、SDS评分低于对照组(P<0.05);观察组干预后POMS量表疲劳、慌乱、紧张、抑郁、易怒评分低于对照组,精力、自尊评分高于对照组(P<0.05)。结论基于综合护理干预可有效减轻局麻术下行腹股沟疝手术患者术中应激反应,使患者积极配合治疗,改善患者焦虑抑郁情绪,缓解不良心境。Objective To explore the effects of comprehensive nursing intervention on perioperative indexes and its improvement effects on psychological status of patients undergoinginguinal hernia surgery under local anesthesia.Methods During the period from January 2018 to July 2019,120 patients who underwent inguinal hernia surgery under local anesthesia in Beijing Chaoyang hospital were randomly divided into observation group and control group,with 60 case in each group.The control group was given routine nursing intervention,whilethe observation group was given comprehensive nursing intervention on basis of control group.The psychological stress status,intraoperative cooperation,anxiety and depression,and mood status were compared between the two groups.Results At 30 minutes before surgery and 30 minutes after entering operating room,levels of blood pressure and heart rate in observation group were lower than those in control group(P<0.05).The scores of intraoperative pain and fear in observation group were lower than those in control group,while score of intraoperative cooperationwas higher than that in control group(P<0.05).After intervention,scores of SAS and SDS in observation group were lower than those in control group(P<0.05).After intervention in POMS,scores of fatigue,panic,nervousness,depression and irritability in observation group were lower than those in control group,while scores of energy and self-esteem were higher than those in control group(P<0.05).Conclusion Comprehensive nursing intervention can effectively reduce intraoperative stress response in patients undergoing inguinal hernia surgery under local anesthesia,make them actively cooperate with treatment,improve their anxiety and depression,and alleviate unhealthy mood.
