糖尿病肾病大鼠治疗后血清Cys-C和Apelin以及钙磷代谢水平变化情况分析

摘　　要：目的探讨糖尿病肾病(DN)大鼠治疗后血清胱抑素C(Cys-C)和APJ内源性配体(Apelin)以及钙磷代谢水平变化情况,并分析其与DN治疗效果的相关性。方法选择6周龄清洁级Wistar大鼠50只为研究对象,其中38只大鼠通过肾切除术联合链脲佐菌素腹腔注射诱导DN模型,剩余12只大鼠仅进行开胸操作,不切除肾脏,并作为假手术组。然后将建模成功的36只大鼠分为3组,各12只。模型组给予等量0.9%氯化钠溶液灌胃,治疗A组给予200 mg/kg二甲双胍灌胃,治疗B组给予200 mg/kg二甲双胍+20 mg/kg氯沙坦灌胃,间隔1 h,假手术组等量0.9%氯化钠溶液灌胃。分别于治疗前1 d、治疗2周和治疗4周,测量各组大鼠体重;酶联免疫吸附试验(ELISA)法检测各组大鼠血清中Cys-C和Apelin水平;罗氏全自动生化仪及配套试剂检测各组大鼠血清钙和磷的水平,并计算钙磷乘积值;采用化学发光法检测各组大鼠血清iPTH水平。并分析DN大鼠治疗效果与大鼠体重、血清Cys-C、Apelin、iPTH水平以及钙磷乘积的相关性。结果治疗1 d至治疗后4周,4组大鼠体重均随时间显著增加,差异均有统计学意义(P<0.05),假手术组和模型组大鼠血清Cys-C、Apelin、钙磷乘积和iPTH水平变化均差异无统计学意义(P>0.05),而治疗A、B组大鼠血清Cys-C、Apelin以及iPTH水平均随时间显著降低,血清钙磷乘积随时间显著升高,差异均有统计学意义(P<0.05)。治疗前1 d,与假手术组相比,模型组、治疗A组、B组大鼠体重、血清Cys-C、Apelin以及iPTH水平均显著增加,钙磷乘积显著降低,差异均有统计学意义(P<0.05),而后三组相比差异无统计学意义(P>0.05)。治疗后2周,与假手术组相比,模型组、治疗A组、B组大鼠体重、血清Cys-C、Apelin以及iPTH水平均显著增加,钙磷乘积显著降低,差异均有统计学意义(P<0.05);与模型组相比,治疗A、B组大鼠体重、血清Cys-C、Apelin以及iPTH水平均显著降低,钙磷乘积显著Objective To investigate the changes of serum Cystatin C(Cys-C),APJ endogenous ligand(Apelin)and calcium and phosphorus metabolism levels in diabetic nephropathy rats after treatment,and to analyze their correlation with the therapeutic effect of diabetic nephropathy.Methods Fifty 6-week-old clean-grade Wistar rats were selected as the research objects,of which 38 rats were nephrectomy combined with intraperitoneal injection of streptozotocin to induce DN model,and the remaining 12 rats only undergo thoracotomy without removing the kidneys,and serve as the sham operation group.Then,the 36 rats that were successfully modeled were divided into 3 groups,each with 12 rats.The model group was given the same amount of normal saline by gavage,the treatment group A was given 200 mg/kg metformin by gavage,and the treatment group B was given 200 mg/kg metformin+20 mg/kg losartan by gavage,1 hour apart.The sham operation group was given an equal amount of 0.9%sodium chloride solution by gavage.The levels of Cys-C and Apelin in serum of rats in each group were detected by enzyme-linked immunosorbent assay(ELISA);the serum calcium and phosphorus levels of rats in each group were tested using the Roche automatic biochemical analyzer and supporting reagents,and the calcium and phosphorus product was calculated;the serum iPTH levels of rats in each group were detected by chemiluminescence method.The correlation between the therapeutic effect of rats with diabetic nephropathy and rat body weight,serum Cys-C,Apelin,iPTH levels,and calcium-phosphorus product was analyzed.Results From 1 d to 4 weeks after treatment,the weight of rats in the four groups increased significantly over time,the differences were statistically significant(P<0.05).There was no significant difference in serum Cys-C,Apelin,calcium-phosphorus product and iPTH levels between the sham operation group and the DN model control group(P>0.05).And the treatment of A and B group rat serum Cys-C,Apelin and iPTH levels decreased significantly over time,and serum calcium
