胸腔镜下肺大泡切除术联合胸膜固定术治疗青少年自发性气胸的疗效观察

摘　　要：目的探讨胸腔镜下肺大泡切除术联合胸膜固定术治疗青少年自发性气胸的效果。方法选择我院2010年1月至2020年2月收治的111例青少年自发性气胸患者,根据治疗方式不同分为对照组(胸腔闭式引流术,51例)与观察组(胸腔镜下肺大泡切除术联合胸膜固定术,60例)。比较两组的临床疗效、临床指标及并发症情况。结果观察组的治疗总有效率为98.33%,高于对照组的82.35%(P<0.05)。观察组的住院时间、留置引流管时间均短于对照组,手术时间长于对照组(P<0.05);观察组的术后并发症发生率为1.67%,低于对照组的15.69%(P<0.05)。结论胸腔镜下肺大泡切除术联合胸膜固定术治疗青少年自发性气胸的效果显著,利于患者术后恢复,且并发症较少。Objective To explore the effect of thoracoscopic bullectomy combined with pleurodesis in the treatment of adolescent patients with spontaneous pneumothorax.Methods 111 adolescent patients with spontaneous pneumothorax admitted to our hospital from January 2010 to February 2020 were selected and divided into control group(chest closed drainage,51 cases)and observation group(thoracoscopic bullectomy combined with pleurodesis,60 cases)according to different treatment methods.The clinical efficacy,clinical indicators and complications were compared between the two groups.Results The total effective rate of treatment in the observation group was 98.33%,higher than 82.35%in the control group(P<0.05).The hospitalization time and indwelling drainage tube time of the observation group were shorter than those of the control group,and the operation time was longer than that of the control group(P<0.05).The incidence of postoperative complications in the observation group was 1.67%,lower than 15.69%in the control group(P<0.05).Conclusions Thoracoscopic bullectomy combined with pleurodesis has significant effect in the treatment of adolescent spontaneous pneumothorax,which is beneficial to postoperative recovery of patients,with fewer complications.
