摘 要:目的分析新生儿PICC导管堵塞的相关影响因素。方法选取2014年2月至2020年5月在我院新生儿科住院期间置入PICC导管的825例新生儿,依据PICC导管是否堵塞分为堵塞组(n=27)与未堵塞组(n=798)。采用单因素和多因素分析影响新生儿PICC导管堵塞的相关因素。结果堵塞组导管末端位置过深、导管留置时间13~40 d、冲管(封管)不当、经导管输注脂肪乳、输液护理不当、持续输液发生率均高于未堵塞组(P<0.05);两组的性别、胎龄、出生时体质量及置管时体质量比较无统计学差异(P>0.05)。Logistic回归分析显示,导管末端位置过深、导管留置时间13~40 d、冲管(封管)不当、经导管输注脂肪乳、输液护理不当、持续输液是造成新生儿PICC导管堵塞的危险因素(OR>1,P<0.05)。结论导管末端位置过深、导管留置时间13~40 d、冲管(封管)不当、经导管输注脂肪乳、输液护理不当、持续输液是导致新生儿PICC导管堵塞发生的危险因素,在临床中需对上述危险因素予以重点关注。Objective To analyze the related influencing factors of neonatal PICC catheter blockage.Methods 825 neonates with PICC catheterization during hospitalization in our department of neonatology from February 2014 to May 2020 were selected and divided into blocked group(n=27)and non-blocked group(n=798)according to whether the PICC catheter blocked or not.Univariate and multivariate analysis were used to analyze the related factors affecting neonatal PICC catheter blockage.Results The incidences of excessive depth of the end of catheter,catheter indwelling time of 13 to 40 days,improper flushing(sealing),fat emulsion infusion through catheter,improper infusion nursing and continuous infusion in the blocked group were higher than those in the non-blocked group(P<0.05).No statistical difference was found in the gender,gestational age,body mass at birth and body mass at catheterization between the two groups(P>0.05).Logistic regression analysis showed that the risk factors for neonatal PICC catheter blockage were excessive depth of the end of catheter,catheter indwelling time of 13 to 40 days,improper flushing(sealing),fat emulsion infusion through catheter,improper infusion nursing and continuous infusion(OR>1,P<0.05).Conclusions Excessive depth of the end of catheter,catheter indwelling time of 13 to 40 days,improper flushing(sealing),fat emulsion infusion through catheter,improper infusion nursing and continuous infusion are the risk factor of neonatal PICC catheter blockage,so it is necessary to pay more attention to the above risk factors in clinical practice.
