沙库巴曲缬沙坦对慢性射血分数降低心力衰竭患者心功能和血管内皮功能的影响研究

摘　　要：目的:观察沙库巴曲缬沙坦对慢性射血分数降低心力衰竭(CHFrEF)患者心功能和血管内皮功能的影响。方法:将笔者所在医院心内科门诊及住院部2019年5-12月收治的80例CHFrEF患者随机分为观察组和对照组,各40例,观察组在规范化治疗的前提下联合沙库巴曲缬沙坦,对照组在规范化治疗的前提下加用培哚普利。两组均持续治疗12周。比较两组治疗前后左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、血流介导血管舒张功能(FMD)、血浆血管紧张素Ⅱ(AngⅡ)、内皮素-1(ET-1)、降钙素基因相关肽(CGRP)和血清一氧化氮(NO)、一氧化氮合酶(NOS)水平等指标变化,比较两组不良反应发生情况。结果:治疗后观察组LVEF、FMD均高于对照组,LVEDD低于对照组,差异均有统计学意义(P<0.05);治疗后观察组AngⅡ、ET-1均低于对照组,CGRP、NO、NOS均高于对照组,差异均有统计学意义(P<0.05)。在治疗过程中,两组均未出现明显不良反应。结论:沙库巴曲缬沙坦能够逐步增强CHFrEF患者的心脏功能,并且还能够进一步增强患者的血管内皮功能。Objective:To observe the effects of Sacubitril Valsartan on cardiac function and vascular endothelial function in patients with chronic heart failure with reduced ejection fraction (CHFrEF).Method:A total of 80 patients with CHFrEF admitted to the outpatient department and inpatient department of the Department of Cardiology of our hospital from May to December 2019 were randomly divided into observation group and control group,with 40 patients in each group.The observation group was treated with Sacubitril Valsartan under the premise of standardized treatment,while the control group was treated with Perindopril under the premise of standardized treatment.Both groups were treated continuously for 12 weeks.The left ventricular ejection fraction (LVEF),left ventricular end-diastolic diameter (LVEDD),flow mediated vasodilation function (FMD),plasma angiotensin A (AngⅡ),endothelin-1 (ET-1),calcitonin gene-related peptide (CGRP),serum nitric oxide (NO) and nitric oxide synthase (NOS) levels were compared between the two groups before and after treatment Indicators change.Result:After treatment,LVEF and FMD in the observation group were higher than those in the control group,while LVEDD in the observation group was lower than those in the control group,with statistical significance (P<0.05).After treatment,AngⅡ and ET-1 in the observation group were lower than those in the control group,while CGRP,NO and NOS were higher than those in the control group,with statistical significance (P<0.05).During the treatment,there were no obvious adverse reactions in the two groups.Conclusion:Sacubitril Valsartan progressively enhances cardiac function in patients with CHFrEF,and further enhances vascular endothelial function.
