小儿脐下手术中喉罩全麻联合骶麻的应用价值研究

摘　　要：目的:探究喉罩全麻与骶麻联合应用在小儿脐下手术中的价值。方法:在本院2019年9月-2020年9月收治的择期行脐下手术患儿中选择120例,对其临床资料进行回顾性分析,并按照麻醉方法不同将患者分成三组,A组(n=40)予气管插管全麻+骶麻,B组(n=40)予喉罩全麻+骶麻肌松,C组(n=40)予喉罩全麻+骶麻非肌松。对三组患儿血流动力学变化、临床指标及副反应发生情况进行比较分析。结果:T2与T4时,B组与C组平均动脉压、心率低于A组,差异有统计学意义(P<0.05);B组与C组拔管或喉罩时间及苏醒时间优于A组,且C组拔管或喉罩时间早于B组,差异均有统计学意义(P<0.05);B组、C组副反应发生率低于A组,差异有统计学意义(P<0.05)。结论:小儿脐下手术中联合采用喉罩全麻与骶麻安全有效,能缩短术后苏醒及拔管时间,减少副反应,值得临床推广。Objective:To explore the value of laryngeal mask airway anesthesia combined with sacral anesthesia in pediatric subumbilical surgery.Method:A total of 120 cases of children undergoing elective subumbilical surgery from September 2019 to September 2020 in our hospital,were selected as the research object,and their clinical data were retrospectively analyzed.According to the different anesthesia methods,the children were divided into three groups:group A (n=40) received general anesthesia with tracheal intubation+sacral anesthesia,group B (n=40) received laryngeal mask anesthesia+sacral anesthesia,group C (n=40) received laryngeal mask anesthesia+sacral anesthesia without muscle relaxant.The hemodynamic changes,clinical indexes and side effects of the three groups were compared and analyzed.Result:At T2 and T4,the average arterial pressure and heart rate of group B and group C were lower than those of group A,the differences were statistically significant (P<0.05);the time of extubation or laryngeal mask airway in group B and group C were better than that in group A,and the time of extubation or laryngeal mask airway in group C was earlier than that in group B,the differences were statistically significant (P<0.05);the incidence of side effects in group B and group C were lower than that in group A,the differences were statistically significant (P<0.05).Conclusion:Laryngeal mask airway combined with sacral anesthesia is safe and effective in pediatric subumbilical surgery,which can shorten the time of postoperative recovery and extubation,and reduce the side effects,which is worthy of clinical promotion.
