后入路减压并行椎弓根内固定治疗腰椎爆裂性骨折伴不全瘫60例

摘　　要：目的:探究后入路减压并行椎弓根内固定治疗腰椎爆裂性骨折伴不全瘫的临床效果。方法:回顾性分析高州市中医院骨伤二科2018年3月-2020年3月收治的腰椎爆裂性骨折伴不全瘫60例临床资料,所有患者均开展后入路减压并行椎弓根内固定治疗。对患者随访6个月,比较术前术后伤椎前缘高度损失率、椎管狭窄程度、Cobb角、神经功能分级及并发症发生情况。结果:手术后伤椎前缘高度损失率、Cobb角明显低于手术前,差异有统计学意义(P<0.05);手术后椎管狭窄程度、神经功能分级明显优于手术前,差异有统计学意义(P<0.05);60例患者术后发生伤口感染2例、压疮1例。结论:后入路减压并行椎弓根内固定治疗腰椎爆裂性骨折伴不全瘫可有效改善伤椎前缘高度损失率、Cobb角,手术后椎管狭窄程度、神经功能恢复良好,并发症少,值得应用与推广。Objective:To explore the clinical effect of posterior approach decompression combined with pedicle internal fixation in the treatment of lumbar burst fracture with incomplete paraplegia.Method:Retrospective analysis was performed on the clinical data of 60 cases of lumbar burst fracture with incomplete paralysis admitted to the second department of bone injury of Gaozhou Hospital of Traditional Chinese Medicine from March 2018 to March 2020.All the patients were treated with posterior approach decompression and pedicle internal fixation.Patients were followed up for 6 months,compared preoperative and postoperative height loss rate of anterior edge of injured vertebra,spinal canal stenosis degree,Cobb angle,neurological function grade and complications.Result:After the operation,the height loss rate of anterior edge of injured vertebra and Cobb angle were significantly lower than those before the operation,and the differences were statistically significant (P<0.05).The degree of spinal canal stenosis and neurological function grade after operation were significantly better than those before operation,the differences were statistically significant (P<0.05).Postoperative wound infection occurred in 2 cases and pressure ulcer in 1 case in 60 patients.Conclusion:Posterior approach decompression combined with pedicle internal fixation in the treatment of lumbar burst fracture with incomplete paraplegia can effectively improving the height loss rate of anterior edge of injured vertebra,Cobb Angle,the degree of spinal canal stenosis,neurological function recovery after surgery is well,fewer complications,it is worthy of application and promotion.
