基于快速康复外科理念的护理策略对计划性剖宫产产妇产后恢复情况的影响

摘　　要：目的观察基于快速康复外科理念的护理策略对计划性剖宫产产妇产后恢复情况的影响。方法将我院收治的352例计划性剖宫产产妇随机分为对照组(常规护理)和观察组(基于快速康复外科理念的护理策略),各176例。比较两组恢复情况、泌乳状况、喂养情况、子宫复旧情况、自护能力及并发症发生情况。结果观察组拔管时间、术后首次排气时间、下床活动时间、疼痛持续时间、住院时间均短于对照组,初乳时间早于对照组,24 h内哺乳次数、术后第2天泌乳量均多于对照组,纯母乳喂养率、子宫复旧良好率高于对照组,产后24、48 h宫底高度均小于对照组,产后并发症总发生率低于对照组(P<0.05)。护理后,两组病情了解度、健康知识水平、自护责任感、自护技能评分均升高,且观察组高于对照组(P<0.05)。结论基于快速康复外科理念的护理策略在计划性剖宫产产妇中的护理效果理想,可促进产妇恢复,提高泌乳功能,促进子宫复旧,提高自护能力,减少产后并发症。Objective To observe the influence of nursing strategy based on the concept of rapid rehabilitation surgery on postpartum recovery of puerperae with planned cesarean section.Methods A total of 352 puerperae with planned cesarean section admitted in our hospital were randomly divided into control group(routine nursing)and observation group(nursing strategy based on the concept of rapid rehabilitation surgery),with 176 cases in each group.The recovery,lactation,feeding,uterine involution,self-care ability and complications of the two groups were compared.Results The extubation time,postoperative first exhaust time,ambulation time,pain duration and hospital stay of the observation group were shorter than those of the control group,the colostrum time was earlier than that of the control group,the number of breast-feeding within 24 h and the amount of lactation on the second day after operation were more than those of the control group,and the pure breast-feeding rate and the good rate of uterine involution were higher than those of the control group,the height of uterine fundus at 24 and 48 h after delivery were smaller than those of the control group,and the total incidence of postpartum complications was lower than that of the control group(P<0.05).After nursing,the scores of understanding degree of illness,health knowledge level,self-care responsibility and self-care skills of the two groups increased,and those of the observation group were higher than the control group(P<0.05).Conclusion The nursing strategy based on the concept of rapid rehabilitation surgery has ideal nursing effect in the puerperae with planned cesarean section,it can promote the recovery of puerperae,improve the lactation function,promote the uterine involution,improve the self-care ability,and reduce the postpartum complications.
