摘 要:目的:评价血管分析软件(VA)对颅内小及微小动脉瘤的诊断价值。方法:收集37例经数字减影血管造影(DSA)确诊的共44个小及微小动脉瘤的蛛网膜下腔出血患者,回顾性分析其头颅CT血管成像(CTA)影像资料。所有影像数据由2组医师独立评估:对照组采用常规容积再现(VR)、最大密度投影(MIP)、多平面重建(MPR)等技术后处理重建并完成诊断;试验组采用常规血管后处理+VA血管重建并完成诊断,比较2组诊断的准确度、灵敏度和特异度。结果:头颅CTA常规后处理重建结合VA对颅内小脑动脉瘤诊断的准确度和效率明显高于常规后处理重建图像诊断。VA对颅内小及微小动脉瘤显示(尤其瘤颈显示)效果优于常规重建。结论:VA软件对颅内小及微小动脉瘤的诊断具有较大临床价值,头颅CTA常规技术结合VA能更好地发现小及微小动脉瘤,快速诊断和鉴别蛛网膜下腔出血的病因,为临床提供诊断依据。Purpose:To evaluate the value of computed tomography angiography(CTA)vessel analysis software(VA)in the diagnosis of small and micro intracranial aneurysms.Methods:Thirty-seven patients with subarachnoid hemorrhage due to ruptured aneurysm(totally 44 small and micro aneurysms)diagnosed by digital subtraction angiography(DSA)were collected.The imaging data of spiral CTA in these patients were analyzed retrospectively.All the data were assessed independently by two experienced radiologists.In the control group,the reconstruction and diagnosis were completed by using conventional volume rendering(VR),maximum intensity projection(MIP),multiple planner reconstruction(MPR)and other technique,while in the test group,the reconstruction and diagnosis were completed by using conventional vascular post-processing plus VA vascular reconstruction technique.The accuracy of diagnosis was compared between the two groups.Results:The accuracy and efficiency of CTA combined with VA in the diagnosis of intracranial cerebellar aneurysms were significantly higher than that of conventional postprocessing.VA was superior to conventional reconstruction in displaying small and micro intracranial aneurysms,especially in the neck of aneurysm.Conclusion:VA has great clinical value in the diagnosis of small and micro intracranial aneurysms.CTA combined with VA can betterly find small and micro intracranial aneurysms,diagnose and differentiate the cause of subarachnoid hemorrhage quickly.
