摘  要:目的分析高海拔地区中青年男性肥胖相关继发性性腺功能减退症(MOSH)特点及影响因素。方法纳入193例高海拔地区中青年男性肥胖者(年龄20~50岁,平均年龄42.16±7.29岁),检测体重、腰围指数和性激素、甲状腺激素、血脂、25-羟维生素D指标,分析MOSH对象的海拔和民族构成,将研究对象分为MOSH组(95例)和单纯中心性肥胖组(98例)并行比较,同时分析高海拔地区MOSH对象的患病危险因素。结果 193例肥胖男性患者中MOSH的构成比为49.22%;MOSH组TG高于单纯中心性肥胖组;MOSH组年龄、雌二醇(E2)及25(OH)D低于单纯中心性肥胖组(P<0.05);两组甲状腺激素水平比较无差异。二元logistic回归分析发现,TG是本地中青年MOSH患者患病的独立危险因素。结论 MOSH在本地呈年轻化趋势;肥胖增加MOSH的患病率,TG是本地中青年MOSH患病的独立危险因素;民族和海拔不是本地中青年MOSH患病危险因素。Objective To analyze the characteristics and influencing factors of obesity-related secondary hypogonadism(MOSH)in young and middle-aged men at high altitude.Methods Body mass index, waist circumference, sex hormone, thyroid hormone, blood lipid and 25-hydroxyvitamin D indexes were measured in 193 obese young and middle-aged men(aged 20~50 years, average aged 42.16±7.29 years)at high altitude.The altitude and ethnic composition of MOSH subjects were analyzed.In addition, the subjects were divided into MOSH group(95 cases)and simple central obesity group(98 cases)for parallel comparison.The risk factors of MOSH subjects at high altitude were also analyzed.Results The composition of MOSH in 193 obese male patients was 49.22%;TG in MOSH group was higher than that in simple central obesity group;age, estradiol(E2)and 25(OH)D in MOSH group were lower than those in simple central obesity group(P<0.05);there was no difference in thyroid hormone levels between the two groups.According to the binary logistic regression analysis, TG was an independent risk factor for local young and middle-aged patients with MOSH.Conclusion More and more local young people are tended to get MOSH.Obesity increases the prevalence of MOSH.TG is an independent risk factor for local young and middle-aged patients with MOSH.Nationality and altitude are not the risk factors for local young and middle-aged patients with MOSH.
