摘　　要：介绍黄世林教授从"骨枯髓虚"论治原发性骨髓纤维化的临床经验。认为本病中医病名可为"骨痿",病理特征为"骨枯髓虚",毒瘀互结、肾虚精亏为本病之基本病机。治疗方面,黄教授形成了具有辨证分期论治特色的治疗方案,将本病分为毒瘀互结证与肾虚精亏证,分别采用解毒清热、化瘀散结法及扶正祛邪、补肾填精法施治,并区分疾病的进展期与稳定期,采用中西医结合治疗。总之,解毒散瘀、扶正祛邪之法应当贯穿本病治疗的始终。This paper introduces Professor Huang Shilin’s clinical experience in treating primary myelofibrosis(PMF) based on the pathological feature of "bone depletion and marrow deficiency".Professor Huang believes that PMF belongs to the category of "Gu Wei"(atrophic debility of bones)in traditional Chinese medicine(TCM), and its pathological feature is "bone depletion and marrow deficiency". The basic pathogenesis is intermingled toxin and blood stasis, kidney asthenia and essence deficiency. Professor Huang has developed a distinctive treatment plan based on syndrome differentiation and staging treatment. He divides PMF into intermingled toxin and blood stasis syndrome and kidney asthenia and essence deficiency syndrome. The former is treated with the method of removing toxin and clearing heat, removing blood stasis and resolving stagnation;while the latter is treated with the method of strengthening body resistance and eliminating pathogenic factors, consolidating kidney and supplementing essence. Professor Huang also differentiates PMF into progressive stage and stable stage, and adopts integrated traditional Chinese and western medicine for treatment. To sum up, the therapeutic principles of removing toxin and resolving stasis, strengthening body resistance and eliminating pathogenic factors should be followed throughout the whole treatment process.
