人工股骨头置换术与全髋关节置换术治疗老年移位型股骨颈骨折的效果分析

摘　　要：目的探讨人工股骨头置换术(HA)与全髋关节置换术(THA)治疗老年移位型股骨颈骨折的效果。方法将2017年1月至2019年12月我院收治的老年移位型股骨颈骨折100例根据手术方法不同分为THA组和HA组,各50例。比较两组患者术后疗效、手术相关指标以及并发症发生率。结果THA组患者的FRS评分、住院费用、手术时间、术中出血量、住院时间显著高于HA组患者,VAS评分显著低于HA组患者(P<0.05)。但两组恢复独立行走率、并发症发生率比较,差异不显著(P>0.05)。结论HA与THA用于治疗老年移位型股骨颈骨折患者效果均理想,其中THA对于改善患者髋部功能效果更好,但是治疗费用比较高。Objective To investigate the effect and prognosis of artificial femoral head(HA)and total hip joint replacement(THA)in the treatment of displaced femoral neck fracture in the elderly.Methods According to different surgical methods,100 cases of elderly displaced femoral neck fractures admitted to our hospital from January 2017 to December 2019 were divided into THA group and HA group,with 50 cases in each.The postoperative curative effect,operation-related indicators and the incidence of complications were compared between the two groups.Results The FRS score,hospitalization cost,operation time,intraoperative blood loss,and hospitalization time of the THA group were significantly higher than those of the HA group,and the VAS score was significantly lower than that of the HA group(P<0.05).However,there was no significant difference in the recovery rate of independent walking and the incidence of complications between the two groups(P>0.05).Conclusion Compared with HA and THA in the treatment of elderly patients with displaced femoral neck fracture,the results are both satisfactory.THA has a good effect on improving the hip function of patients,but the treatment cost is relatively high.
