摘　　要：目的探究重度妊娠高血压综合征患者行剖宫产术的围麻醉期处理和对母婴结局的影响。方法选取我院产科2017年4月至2019年4月接收的90例行剖宫产术的重度妊娠高血压综合征患者作为研究对象。将所有患者随机分为试验组和对照组,各45例。对照组患者给予硬膜外麻醉,试验组患者给予腰麻联合硬膜外麻醉。对比两组患者的麻醉效果、不良反应发生情况、新生儿健康情况等。结果试验组患者的麻醉维持时间比对照组短,低血压发生率、不良反应发生率比对照组低,差异有统计学意义(P<0.05)。两组麻醉阻滞平面达到情况、平均动脉压(mean arterial pressure,MAP)水平相比,差异无统计学意义(P>0.05)。试验组新生儿健康情况好于对照组,差异有统计学意义(P<0.05)。结论重度妊娠高血压综合征患者行剖宫产术的风险较高,因此应予以适当的麻醉方式。相较于单纯的硬膜外麻醉,腰麻联合硬膜外麻醉的效果更佳,不良反应更少,新生儿健康情况更好。Objectives To investigate the effect of cesarean section on the treatment of cesarean section in patients with severe pregnancy-induced hypertension and the effect on maternal and child outcomes.Methods A total of 90 patients with severe pregnancy-induced hypertension who underwent cesarean section received from April 2017 to April 2019 in our hospital were selected as subjects.All patients were randomly divided into the experimental group and the control group,with 45 cases in each group.Epidural anesthesia was given to the control group,and spinal anesthesia combined with epidural anesthesia was given to the experimental group.The anesthetic effects,adverse reactions,and neonatal health of the two groups were compared.Results The anesthesia maintenance time of the experimental group was shorter than that of the control group,the incidence of hypotension and adverse reactions were lower than the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference between the two groups in the anesthesia block level and mean arterial pressure(MAP)level(P>0.05).The health status of the newborns in the experimental group was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Patients with severe pregnancy-induced hypertension have a higher risk of cesarean section,so appropriate anesthesia should be given.Compared with simple epidural anesthesia,spinal anesthesia combined with epidural anesthesia is better.The adverse reactions is lower,and the neonatal health is better.
