无肌松靶控输注麻醉在输尿管镜下钬激光碎石术中的应用

摘　　要：目的探讨无肌松靶控输注麻醉在输尿管镜下钬激光碎石术中的安全性和可行性。方法选择择期行输尿管镜下钬激光碎石术成人患者80例,随机分为肌松全身麻醉组(A组)和无肌松全身麻醉组(B组)各40例。观察两组麻醉效果。结果与T_(0)比较,T_(1)两组SBP、MAP和HR均有所降低,B组比A组明显降低(P<0.05);T_(2)、T_(3)时间点两组SBP、MAP和HR变化无明显差异(P>0.05);T_(4)、T_(5)时间点两组SBP和MAP均有所升高,且B组比A组明显升高(P<0.05),HR差异无显著意义。B组麻醉不良反应总发生率高于A组(P<0.05)。结论无肌松靶控输注麻醉应用于输尿管镜下钬激光碎石手术安全可行,能达到有效的镇静镇痛强度,不良反应风险低。Objective To investigate the safety and feasibility of target controlled infusion anesthesia without muscle relaxant in ureteroscopic holmium laser lithotripsy.Methods There were 80 adult patients selected for ureteroscopic holmium laser lithotripsy,which were randomly divided into 40 cases in the general anesthesia group(group A)and 40 cases in the non-muscle relaxant general anesthesia group(group B).Observe the anesthesia effect of the two groups.Results Compared with T_(0),the SBP,MAP and HR of the two groups of T_(1) were reduced,group B was significantly lower than that of group A(P<0.05);there was no significant difference in the changes of SBP,MAP and HR between the two groups at T_(2) and T_(3)(P>0.05);SBP and MAP of both groups increased at T_(4) and T5,and group B was significantly higher than group A(P<0.05),and there was no significant difference in HR.The total incidence of adverse anesthesia reactions in group B was higher than that in group A(P<0.05).Conclusion It is safe and feasible to use non-muscle release target controlled infusion anesthesia in ureteroscopic holmium laser lithotripsy,which can achieve effective sedative and analgesic intensity and low risk of adverse reactions.
