两种不同供皮区带蒂皮瓣移植在手部皮肤缺损修复中的疗效

摘　　要：目的探析在手部皮肤缺损修复患者手术治疗过程中两种不同供皮区带蒂皮瓣移植的临床效果。方法本次试验的研究对象为接受手部皮肤缺损修复手术治疗的58例患者,研究起止时间为2017年2月至2019年2月,按照患者供皮区选择的不同分为两组,将皮瓣移植供皮区选择第1掌骨桡背侧的29例患者作为A组,将皮瓣移植供皮区选择示指邻指的29例患者作为B组。比较两组的治疗效果。结果A组患者与B组患者在皮瓣成活率、新生皮肤与正常皮面相差4 mm发生率、瘢痕痉挛发生率以及手运动功能恢复优良率比较,差异无统计学意义(P>0.05)。结论在为手部皮肤缺损患者实施修复手术的过程中,将第1掌骨桡背侧以及示指邻指作为带蒂皮瓣移植的供皮区均有相对较为理想的效果,皮瓣成活率高,且出现新生皮肤与正常皮面相差4 mm、瘢痕痉挛的可能性较小,对患者手外观以及运动功能的恢复有着积极的影响,因此可将第1掌骨桡背侧以及示指邻指作为带蒂皮瓣移植的理想供皮区。Objective To explore the clinical effect of pedicled skin flap transplantation from two different skin donor regions in the surgical treatment of patients with hand skin defect repair.Methods The subjects of this experimental study were 58 patients who received hand skin defect repair surgery.The research start and end time was between February 2017 and February 2019.They were divided into two groups according to the different selection of skin donor area.The skin donor area of skin flap transplantation selected 29 patients on the radial dorsal side of the first metacarpal bone as group A,and the skin donor area of skin flap transplantation selected 29 patients on the adjacent finger as group B.Compare the treatment effects of the two groups.Results There was no significant difference between group A and group B in the survival rate of skin flap,the incidence rate of 4 mm difference between newborn skin and normal skin surface,the incidence rate of scar spasm and the excellent recovery rate of hand motor function(P>0.05).Conclusion In the process of repair surgery for patients with hand skin defects,the radial dorsal side of the first metacarpal bone and the adjacent finger of the index finger are both relatively ideal skin donor areas for pedicled skin flap transplantation.The survival rate of the flap is high,the difference between the new skin and the normal skin surface is 4 mm,the possibility of scar spasm is small,and it has a positive impact on the recovery of the patient's hand appearance and motor function.Therefore,the radial dorsal side of the first metacarpal bone and the adjacent finger of the index finger can be used as ideal skin donor areas for pedicled skin flap transplantation.
