椎管内麻醉和全身麻醉在老年骨创伤患者中的临床疗效

摘　　要：目的分析椎管内麻醉和全身麻醉在老年骨创伤患者中的临床疗效。方法选取2018年4月至2019年4月我院收治的选择椎管内麻醉的老年骨创伤患者43例作为椎管组,选取同期我院收治的选择全身麻醉的老年骨创伤患者43例作为全身组,对两组患者的相关指标进行分析与比较。结果椎管组患者睁眼时间、语言陈诉时间均短于全身组患者,麻醉药用量少于全身组患者,差异有统计学意义(P<0.05);椎管组患者麻醉后3、6、12 h的认知功能评分明显高于全身组患者(P<0.05);椎管组患者麻醉后3、6、12 h的认知功能障碍发生率明显低于全身组患者(P<0.05)。结论针对老年骨创伤患者,椎管内麻醉的效果相对于全身麻醉来说更为理想,可稳定老年患者的精神状态,减少认知功能障碍的出现,且其睁眼时间、语言陈诉时间短,麻醉药物用量少。Objective To analyze the clinical effects of intraspinal anesthesia and general anesthesia in the process of anesthesia for elderly patients with bone trauma.Methods In this experimental study,43 elderly patients with bone trauma who chose intraspinal anesthesia as the anesthesia method and 43 elderly patients with bone trauma who chose general anesthesia as the anesthesia method in our hospital from April 2018 to April 2019 were selected as the spinal canal group,and 43 elderly patients with bone trauma who chose general anesthesia as the anesthesia method were selected as the systemic group,and the relevant indexes of the two groups of patients were analyzed and compared.Results Patients in the spinal canal group had shorter eye-opening time and language reporting time than those in the systemic group,and the consumption of anesthetics was less than that of the systemic group,the differences were statistically significant(P<0.05);the cognitive function scores of patients in the spinal canal group at 3,6,and 12 hours after anesthesia were significantly higher than those in the systemic group(P<0.05);the incidence of cognitive dysfunction in the spinal canal group was significantly lower than that of the systemic group at 3,6,and 12 hours after anesthesia(P<0.05).Conclusion For elderly patients with bone trauma,the effect of intraspinal anesthesia is more ideal than general anesthesia.It can stabilize the mental state of elderly patients,reduce the appearance of cognitive dysfunction,and has a short eye-opening time and language reporting time.The amount of medicine is small.
