应用BK8838超声探头进行超声排粪造影对盆底疾病的辅助诊断价值分析

摘　　要：目的分析应用BK8838超声探头进行超声排粪造影(EDF)在盆底疾病诊断中的效果。方法纳入2018年11月至2020年11月就诊于南京中医药大学第二附属医院肛肠科的137例盆底疾病患者为研究对象,118例存在功能性便秘症状,19例存在功能性肛门直肠痛症状,6例存在肛门失禁症状。所有患者均接受EDF、肛管直肠压力测定及X线排粪造影检查,记录检查结果。结果有116例在EDF诊断为盆底失弛缓,同时肛管直肠压力测定结果支持该诊断,其中114例在X线排粪造影得出相同诊断;在两种排粪造影检查均诊断为直肠前突、盆底疝者分别为38例、3例;有34例存在功能性便秘症状及4例存在功能性肛门直肠痛症状的患者在EDF诊断为直肠黏膜内套叠,其中10例存在功能性便秘症状的患者在X线排粪造影得出相同诊断。在6例存在肛门失禁症状的患者中,EDF均提示存在肛门括约肌缺损,X线排粪造影则均未见明显异常。结论对盆底疾病患者以BK8838探头进行EDF所获取图像质量理想,操作可重复性好,可良好反映盆底运动的动态过程,具有较好的临床应用前景。Objectives To investigate the value of echodefecography with BK8838 ultrasound probe for diagnosing pelvic floor diseases.Methods One hundred and thirty-seven patients with pelvic floor diseases were recruited from the Department of Colorectal Surgery,The Second Affiliated Hospital of Nanjing University of Chinese Medicine between November 2018 and November 2020.One hundred and eighteen patients had functional constipation symptoms,nineteen had functional anorectal pain symptoms,and six had fecal incontinence symptoms.All patients received echodefecography,anorectal manometry,and X-ray defecography,and their results were recorded.Results Using echodefecography,116 patients were diagnosed with pelvic floor achalasia,all of which were confirmed by anorectal manometry,and 144 of which were confirmed by X-ray defecography.Thirty-eight patients were diagnosed with rectal protrusion and three with pelvic floor hernia,both using X-ray defecography and using echodefecography.Thirty-four patients with functional constipation symptoms and four with functional anorectal pain symptoms were diagnosed with intrarectal invaginate using echodefecography,and ten of which with functional constipation symptoms were diagnosed with intrarectal invaginate using X-ray defecography.Echodefecography showed sphincter injuries among the six patients with fecal incontinence symptoms,while X-ray defecography did not show any abnormality among these patients.Conclusion In patients with pelvic floor diseases,echodefecography with BK8838 ultrasound probe could obtain high-quality images and reveal the dynamics of pelvic floor movements.It was a reproducible procedure and had the potential for broader application in clinical practice.
