内痔与混合痔“四因素”评估方案的建立及临床应用价值的探讨

摘　　要：对痔病进行科学、规范的评估是制定治疗方案的基础,一个好的痔评估方案不仅应有助于制定有效的治疗决策,还应有助于科学地比较治疗结果。本文分析了痔评估方案的研究现状,对当前应用最广泛的Goligher分类法所存在的不足进行探讨。通过复习相关文献并结合临床实践经验,在痔的临床症状和体征中筛选出痔的脱垂程度、痔病的出血症状、痔核占据肛管环周的比例、混合痔的外痔部分类型共四个因素对内痔、混合痔进行评估,形成基于“四因素”的痔评估方案,并介绍其临床应用方法。Scientific and standardized assessment of hemorrhoids is the basis for developing treatment strategies.A good assessment can facilitate treatment decision and comparison of treatment effects.In this review,we present the current status of assessment for hemorrhoids.We analyzed the shortcomings of the most widely used Golighers classification.By analyzing current literature and our own clinical experiences,we proposed a novel four-factor assessment.It is based on four symptoms and clinical presentations of hemorrhoids,namely the degree of prolapse,bleeding,the proportion of anal canal circumference occupied,and the types of external hemorrhoids if it is mixed hemorrhoids.We present the application of this new assessment for internal and mixed hemorrhoids in clinical practice.
