摘  要:目的探讨自制伤椎复位装置辅助经伤椎置钉短节段内固定治疗Magerl A3型胸腰椎骨折的临床疗效。方法回顾性分析自2017-05—2019-02采用自制伤椎复位装置辅助经伤椎置钉短节段内固定治疗的21例Magerl A3型胸腰椎骨折,比较术前、术后3 d及末次随访时的伤椎前缘高度压缩率、伤椎中央高度压缩率、椎管占位率、伤椎后凸Cobb角。结果 21例均顺利完成手术并获得完整随访,随访时间11~19个月,平均13.9个月。未出现螺钉松动、拔出及钉棒断裂等内固定失败并发症。术后3 d与末次随访时伤椎前缘高度压缩率、伤椎中央高度压缩率、椎管占位率、伤椎后凸Cobb角较术前均明显减小,差异有统计学意义(P<0.05)。结论自制伤椎复位装置辅助经伤椎置钉短节段内固定治疗Magerl A3型胸腰椎骨折能有效恢复伤椎高度,改善椎管占位并矫正后凸畸形。Objective To investigate the clinical effect of posterior short-segment fixation in combination with intermediate screws for thoracolumbar Magerl A3 type fractures by a self-devised injured vertebra reduction instrument.Methods From May 2017 to February 2019,21 patients underwent posterior short-segment fixation in combination with intermediate screws for thoracolumbar Magerl A3 type fractures by a self-devised injured vertebra reduction instrument.The anterior height(Ha) compression ratios,middle height(Hm) compression ratios,occupation ratios of spinal canal and the kyphotic Cobb angles of injured vertebra were calculated and compared before operation,3 days after operation and at final follow-up.Results All the 21 cases successfully completed the operation and received complete follow-up.The follow-up time was 11-19 months,with an average of 13.9 months.The loosening,displacement and breakage of screws and rids were not observed during the follow-up.The Ha compression ratio,Hm compression ratio,occupation ratio of spinal canal and the kyphotic Cobb angle at 3 days after operation and final follow-up were significantly lower than those before operation(P<0.05).Conclusion Posterior short-segment fixation in combination with intermediate screws for thoracolumbar Magerl A3 type fractures by the self-devised injured vertebra reduction instrument can restore the height of the injured vertebral body,relieve the occupation of spinal canal,and reduce the local kyphotic Cobb angle effectively.
