Hcy在急性心肌梗死伴充血性心力衰竭中的表达及相关性分析

摘　　要：目的探讨同型半胱氨酸(Hcy)在急性心肌梗死与急性心肌梗死伴充血性心力衰竭中的表达及相关性。方法选取2017年1月~2019年12月在本院就诊的急性心肌梗死患者172例为研究对象,根据是否伴发充血性心力衰竭分为合并组72例与单纯组100例。采用超声心动图测定左心室舒张末内径(LVEDD)、左室射血分数(LVEF),检测血清Hcy表达水平并进行相关性分析及影响因素分析。结果合并组LVEDD水平高于单纯组,LVEF水平低于单纯组,差异有统计学意义(P<0.05);合并组血清Hcy水平高于单纯组,差异有统计学意义(P<0.05);充血性心力衰竭Hcy水平与LVEDD水平呈正相关,与LVEF水平呈负相关(P<0.05);多因素Cox回归分析结果显示,Hcy、LVEF、LVEDD为导致充血性心力衰竭发生的独立影响因素(P<0.05)。结论急性心肌梗死伴充血性心力衰竭中的LVEDD水平较高,LVEF水平较低,Hcy在急性心肌梗死伴充血性心力衰竭中呈现高表达状况,并且与患者的心功能有相关性,是导致急性心肌梗死伴充血性心力衰竭发生的重要因素。Objective To investigate the expression and correlation of homocysteine(Hcy)in acute myocardial infarction and acute myocardial infarction with congestive heart failure.Methods A total of 172 patients with acute myocardial infarction who were treated in our hospital from January 2017 to December 2019 were selected as the research objects.According to whether they were accompanied by congestive heart failure,they were divided into a combined group of 72 cases and a simple group of 100 cases.The left ventricular end-diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF)were measured by echocardiography,the serum Hcy expression level was detected,and the correlation analysis and influencing factors were analyzed.Results The LVEDD level of the combined group was higher than that of the simple group,and the LVEF level was lower than that of the simple group,the difference was statistically significant(P<0.05);The serum Hcy level of the combined group was higher than that of the simple group,the difference was statistically significant(P<0.05);Congestive heart failure Hcy level was positively correlated with LVEDD level,and negatively correlated with LVEF level(P<0.05);The results of multivariate Cox regression analysis showed that Hcy,LVEF and LVEDD were independent influencing factors of congestive heart failure(P<0.05).Conclusion In acute myocardial infarction with congestive heart failure,the level of LVEDD is higher,and the level of LVEF is lower.Hcy is highly expressed in acute myocardial infarction with congestive heart failure,and is related to the patient’s heart function.It is an important factor leading to acute myocardial infarction with congestive heart failure.
