摘 要:目的探讨自发性脑出血患者的血清神经元特异性烯醇化酶(NSE)和髓鞘碱性蛋白(MBP)的水平变化,并根据美国国立卫生院神经功能缺损评估量表(NIHSS)进行相关性研究分析。方法选取广州中医药大学第一附属医院颅脑外科自2017年8月-2019年8月收治的脑出血患者120例,根据NIHSS评分将患者分为四组,各30例,其中NIHSS在0~1分之间的为对照组,NIHSS评分在2~15分之间的为轻度组,NIHSS评分在16~20分之间的为中度组,NIHSS评分在21~42分之间的为重度组,对症治疗并通过酶联免疫吸附法进行NSE和MBP不同时间点的动态检测。比较不同功能障碍患者两个指标的变化趋势,分析两个指标与NIHSS水平的相关性。结果轻、中、重度组三种不同的组别的患者血清NSE和MBP水平显著高于对照组,差异具有统计学意义(P<0.05)。针对轻、中、重度组间不同时间段的NSE及MBP水平变化比较,差异有统计学意义(P<0.05),且重度组NSE、MBP浓度显著增高。患者血清NSE、MBP水平与NHISS评分均呈正相关(r=0.484,0.447,P<0.05)。结论自发性脑出血患者血清NSE和MBP水平和神经功能障碍程度密切相关,并动态监测有助于评估患者病情演变和神经功能恢复的程度。Objective To investigate the changes of serum neuron specific enolase(NSE)and myelin basic protein(MBP)levels in patients with spontaneous cerebral hemorrhage,and to analyze the correlation according to NIHSS.Methods 120 patients with cerebral hemorrhage who were admitted to the First Affiliated Hospital of Guangzhou University of traditional Chinese medicine from August 2017 to August 2019 were divided into control group(30 cases,NIHSS 0~1),mild group(30 cases,NIHSS 2-15),moderate group(30 cases,NIHSS 16~20)and severe group(30 cases,NIHSS 21~42)according to NIHSS score The dynamic detection of NSE and MBP at different time points was performed by enzyme-linked immunosorbent assay.The change trend of two indexes in patients with different dysfunction was compared,and the correlation between the two indexes and NIHSS level was analyzed.Results The levels of NSE and MBP in the three groups were significantly higher than those in the control group(P<0.05).Compared with the changes of NSE and MBP levels in different time periods,the difference was obviou(P<0.05),and the concentration of NSE and MBP in severe group was obvious higher.There was a positive correlation between NSE,MBP and nhiss(r=0.484,0.447,P<0.05).Conclusion The levels of NSE and MBP in patients with spontaneous cerebral hemorrhage are closely related to the degree of neurological dysfunction,and dynamic monitoring is helpful to evaluate the degree of disease evolution and neurological function recovery.
