益气活血化痰方治疗慢性阻塞性肺疾病稳定期疗效研究

摘　　要：目的:探讨益气活血化痰方治疗慢性阻塞性肺疾病(COPD)稳定期气虚痰瘀证的临床疗效。方法:选取80例COPD稳定期气虚痰瘀证患者,随机分为对照组40例、治疗组40例。对照组给予羧甲司坦片联合噻托溴铵粉吸入粉雾剂治疗,治疗组在对照组基础上给予益气活血化痰方。比较两组临床疗效、中医症状评分、改良英国医学研究会呼吸困难指数(mMRC)评分、COPD评估测试(CAT)、肺功能、血气指标、不良反应。结果:治疗后,两组患者中医症状评分、mMRC评分、CAT均较治疗前下降,且治疗组低于对照组(均P<0.05)。治疗后,两组患者FEV1、FVC、FEV1/FVC均较治疗前升高,且治疗组水平高于对照组(均P<0.05)。治疗后,两组患者PaO_(2)、SaO_(2)均较治疗前升高,PaCO_(2)较治疗前降低,治疗组PaO_(2)、SaO_(2)水平高于对照组(均P<0.05)。治疗组总有效率95.00%高于对照组的77.50%(P<0.05)。结论:益气活血化痰方治疗COPD稳定期气虚痰瘀证患者疗效较好,可增强肺功能、改善患者生活质量。Objective:To study the clinical effect of Yiqi Huoxue Huatan decoction in treating stable stage of chronic obstructive pulmonary disease(COPD)of Qixu Tanyu type.Methods:80 patients with COPD stable stage of Qixu Tanyu type were randomly divided into control group 40 cases and treatment group 40 cases.The control group was treated with carboxymethyl stann tablet combined with tiotropium bromide powder inhalation powder aerosol,and the treatment group was treated with Yiqi Huoxue Huatan recipe on the basis of the control group.The clinical efficacy,TCM symptom score,modified British medical research council(mMRC)score,COPD assessment test(CAT),pulmonary function index and blood gas index,adverse reactions were compared between the two groups.Results:After treatment,the TCM symptom score,mMRC score and CAT of the two groups decreased,and the treatment group were lower than the control group(all P<0.05).After treatment,The FEV1,FVC,FEV1/FVC of the two groups were higher than those before treatment,and the treatment group were higher than the the control group(all P<0.05).After treatment,the PaO_(2),SaO_(2)of the two groups were higher than those before treatment,the PaCO_(2)was lower than that before treatment,and the PaO_(2),SaO_(2)of the treatment group were higher than those of the control group(all P<0.05).The total effective rate of the treatment group was 95.00%significantly higher than 77.50%of the control group(P<0.05).Conclusion:Yiqi Huoxue Huatan recipehas good effect on patients with COPD in stable period of Qixu Tanyu type,which can enhance lung function and improve the quality of life of patients.
