摘　　要：目的:探究心电图Sokolow、Cornell指数与妊娠期高血压性心脏病的相关性。方法:将疑似妊娠期高血压性心脏病患者135例作为研究对象。检测并记录所有研究对象心电图Sokolow、Cornell指数,分析两者与妊娠期高血压性心脏病的相关性。结果:135例患者中,经检查确诊为妊娠期高血压性心脏病患者60例,排除患者75例。Sokolow指数检查诊断妊娠期高血压性心脏病患者56例,其中假阳性患者10例,其灵敏度76.67%,特异度86.67%,准确度82.22%,Kappa值0.638。Cornell指数检查诊断妊娠期高血压性心脏病患者65例,其中假阳性患者20例,其灵敏度75.00%,特异度73.33%,准确度74.07%,Kappa值0.479。Sokolow、Cornell指数联合检查诊断妊娠期高血压性心脏病患者68例,其中假阳性患者11例,其灵敏度95.00%,特异度85.33%,准确度89.63%,Kappa值0.793。Sokolow、Cornell指数联合诊断的灵敏度高于两者单项诊断,准确度高于Cornell指数(均P<0.05)。结论:心电图Sokolow、Cornell指数联合可显著提高妊娠期高血压性心脏病的诊断灵敏度及准确度。Objective:To explore the correlation between electrocardiogram Sokolow,Cornell indexes and hypertensive heart disease during pregnancy.Methods:135 patients with suspected hypertensive heart disease during pregnancy were selected as the research objects.ECG Sokolow and Cornell indexes were measured and recorded,and the correlation between them and hypertensive heart disease during pregnancy was analyzed.Results:Among 135 patients,60 cases were diagnosed as hypertensive heart disease,75 cases were excluded.The Sokolow index diagnosis showed that 56 patients had hypertensive heart disease during pregnancy,of which 10 cases was false positive,the sensitivity,specificity,accuracy and Kappa value of Sokolow index were 76.67%,86.67%,82.22%and 0.638.The Cornell index diagnosis showed that 65 patients had hypertensive heart disease during pregnancy,of which 20 cases was false positive,the sensitivity,specificity,accuracy and Kappa value of Cornell index were 75.00%,73.33%,74.07%and 0.479.The Sokolow index combined with Cornell index diagnosis showed that 68 patients had hypertensive heart disease during pregnancy,of which 11 cases was false positive,the sensitivity,specificity,accuracy and Kappa value of Sokolow and Cornell indexes were 95.00%,85.33%,89.63%and 0.793,respectively.The sensitivity of combined diagnosis was significantly higher than that of single diagnosis,and the accuracy was significantly higher than diagnosis of Cornell index(all P<0.05).Conclusion:The combination of Sokolow and Cornell indexes can significantly improve the sensitivity and accuracy of diagnosis of hypertensive heart disease in pregnancy.
