摘 要:目的:为高肺血流性肺动脉高压的机制研究建立更稳定的实验动物模型。方法:将60只SD大鼠随机分为假手术组、常规分流组、改良分流组,每组20只。假手术组予开腹夹闭腹主动脉5 min;常规分流组和改良分流组均行腹主动脉—下腔静脉穿刺分流术。对穿刺分流术的穿刺口缝合方式进行改良,常规分流组穿刺口予荷包缝合,改良分流组对穿刺口行四点勾线打结缝合。建模11周后,对大鼠腹主动脉—下腔静脉瘘口行B超探查,评估瘘口形成情况;测右心室压力及右心室肥厚指数,评估成模情况;剖腹探查血管肉眼下大体解剖情况。结果:改良分流组大鼠存活率(95%)明显高于常规分流组(65%)(P<0.05);改良分流组瘘口狭窄率(0%)明显低于常规分流组(36.4%)(P<0.05);改良分流组成模率(90%)高于常规分流组(40%)(P<0.05);改良分流组和常规分流组右心室压力和右心室肥厚指数均增高,其中改良分流组较常规分流组右心室压力升高明显(P<0.05)。结论:腹主动脉—下腔静脉穿刺分流术穿刺口缝合方式的改良,使大鼠存活率提高、瘘口狭窄率降低、成模率提高,右心室压力升高更明显,更有利高肺血流性肺动脉高压的机制的研究。Objective:To establish a more stable experimental animal model for the study of the mechanism of pulmonary hypertension induced by high pulmonary blood flow.Methods:Sixty SD rats were randomly divided into sham operation group,conventional shunt group and modified shunt group,with 20 rats in each group.In sham operation group,abdominal aorta was clamped for 5 minutes.Abdominal aorta inferior vena cavashunt was performed in both conventional shunt group and modified shunt group.The conventional shunt group was treated with purse string suture,and the modified shunt group was treated with four point suture.After eleven weeks of modeling,the abdominal aorta inferior vena cava fistulas were detected by B-ultrasound to evaluate the formation of fistulas.Right ventricular pressure and right ventricular hypertrophy index were measured to evaluate the model formation.The gross anatomy of blood vessels was explored by laparotomy.Results:The survival rate of modified shunt group(95%)was significantly higher than that of conventional shunt group(65%)(P<0.05).The fistula stenosis rate of modified shunt group(0)was significantly lower than that of conventional shunt group(36.4%)(P<0.05).The model rate of modified shunt group(90%)was higher than that of conventional shunt group(40%)(P<0.05).The right ventricular pressure and right ventricular hypertrophy index were increased in the modified shunt group and the conventional shunt group,and the right ventricular pressure in the modified shunt group was significantly higher than that in the conventional shunt group(P<0.05).Conclusion:The improved suture method of abdominal aorta inferior vena cava puncture shunt can improve the survival rate of rats,reduce the stenosis rate of fistula and improve the modeling rate,and the increase ofincrease the right ventricular pressure is more obvious,which is more beneficial to the mechanism research of pulmonary hypertension with high pulmonary blood flow.
