摘  要:目的建立高效液相色谱法测定人血清中替考拉宁血药浓度,用于临床血药浓度监测,指导临床合理用药。方法色谱柱为Hypersil C_(18)柱(250 m m×4.6 mm,5μm),流动相为乙腈-0.01 mol·L^(-1)NaH_(2)PO_(4)(25∶75,v/v,H_(3)PO_(4)调pH值为2.1),流速为1.0 mL·min^(-1),柱温为25℃,检测波长为240 n m。结果替考拉宁在4~100 mg·L^(-1)范围内线性关系良好(r=0.9985),定量下限为4 mg·L^(-1),日内、日间精密度RSD均小于15%,提取回收率85.0%~88.1%,方法回收率96.9%~100.9%。测定31例替考拉宁用药患者血药谷浓度,达标率为41.9%。结论该方法操作简便,检测灵敏、准确,可满足临床替考拉宁血药浓度监测需要。临床实际应用中替考拉宁谷浓度达标率低,个体差异大,建议通过血药浓度监测来保证用药安全有效。OBJECTIVE To establish a HPLC method for the determination of teicoplanin in human serum,which can be used to monitor teicoplanin serum concen tration forrational administration.METHODS HPLC analysis was performed using a Hypersil C18 column(250 mm×4.6 mm,5μm).The mobile phase was acetonitrile-0.01 mol·L^(-1) NaH_(2)PO_(4)(25∶75,v/v,pH was ad justed to 2.1 with H3PO4).The flow rate was 1.0 mL·min^(-1),the temperature of column was 25℃,and the detection wavelength was 240 nm.RES ULTS Teicoplanin had a good linear relationship in the range of 4-100 mg·L^(-1)(r=0.9985).The minimum limit of quantitation was 4 mg·L^(-1).The RSD of intra-day and inter-day were both less than 15%.The extractionre covery was 85.0%-88.1%,and the method recovery was 96.9%~100.9%.The comp liance rate of teicoplanin trough serum concentration was 41.9%in 31 patients.CONCLUSION The method is simple,sensitive and accurate,which can be used for the determination of teicoplanin in serum.In clinical practice,the compliance rate of teicoplanin trough serum concentration is low and the individual difference is large.It is recommended to monitor teicoplanin trough serum concentration to ensure the safety and effectiveness ofmedication.
