经阴道超声检测绒毛滋养动脉血流参数水平在先兆流产妊娠结局中的预测价值分析

摘　　要：目的:采用经阴道超声检测绒毛滋养动脉血流参数水平,并探讨该指标在预测先兆流产妊娠结局中的临床价值。方法:选取某院收治的60例自2019年1月~2020年12月的先兆流产孕妇作为研究对象,均行经阴道超声检查绒毛滋养动脉血流参数,并随访至妊娠16周,根据随访结果分为妊娠组(n=38)和流产组(n=22),比较两组间绒毛滋养动脉血流参数水平。结果:流产组孕妇S/D、RI水平高于妊娠组(P<0.05),而PI水平两组间比较无明显差异(P>0.05);流产组孕妇VI、FI、VFI水平均高于妊娠组(P<0.05);采用ROC曲线评价各参数水平对先兆流产妊娠结局的预测价值,结果显示RI、FI、VFI具有较高的预测价值,曲线下面积分别为0.834、0.865、0.786(P<0.05)。结论:采用经阴道超声检测绒毛滋养动脉RI、FI、VFI水平,对先兆流产妊娠结局具有一定的预测价值。Objective:To investigate the clinical value of blood flow parameters of trophoblastic artery detected by transvaginal ultrasound(TVS)in predicting pregnancy outcome of threatened abortion.Methods:60 pregnant women with threatened abortion in a hospital from January 2019 to December 2020 were selected as the research objects,all of them underwent transvaginal ultrasound to check the blood flow parameters of villous trophoblastic artery,and were followed up to 16 weeks of gestation.According to the follow-up results,they were divided into pregnancy group(n=38)and abortion group(n=22),and the blood flow parameters of villous trophoblastic artery between the two groups were compared.Results:The levels of S/D and RI in the abortion group were higher than those in the pregnancy group(P<0.05),while there was no significant difference in the levels of PI between the two groups(P>0.05).The levels of VI,FI and VFI in the abortion group were higher than those in the pregnancy group(P<0.05).The ROC curve was used to evaluate the predictive value of each parameter level on the pregnancy outcome of threatened abortion.The results showed that RI,FI and VFI had higher predictive value,and the areas under the curve were 0.834,0.865,0.786,respectively(P<0.05).Conclusion:Using transvaginal ultrasound to detect RI,FI and VFI levels of villous trophoblastic artery has a certain predictive value for pregnancy outcome of threatened abortion.
