2型糖尿病周围神经病变患者神经肌电图分析

摘　　要：目的分析90例2型糖尿病(T2DM)周围神经病变患者的神经肌电图临床特征。方法抽取聊城市第二人民医院2019年1月至2019年12月收治的90例T2DM患者作为研究对象,根据T2DM患者是否合并周围神经病变情况分为合并周围神经病变组(28例)与未合并周围神经病变组(62例)。所有患者均进行神经肌电图检查,统计并分析两组患者正中神经运动传导速度(MCV)、腓总神经MCV,正中神经感觉传导速度(SCV)、腓浅神经SCV。绘制受试者工作特征曲线(ROC曲线),观察曲线下面积(AUC),分析神经肌电图检查诊断T2DM周围神经病变的价值。结果合并周围神经病变组患者的MCV正中神经、MCV腓总神经、SCV正中神经、SCV腓浅神经均低于未合并周围神经病变组患者,差异有统计学意义(P<0.05)。绘制ROC曲线,结果显示MCV正中神经、MCV腓总神经、SCV正中神经、SCV腓浅神经以及联合检测诊断周围神经病变AUC=0.764、0.727、0.809、0.746、0.908,其中联合检测的评估价值较好。结论T2DM周围神经病变患者的神经肌电指标明显升高,且神经肌电图检查对周围神经病变的诊断效果较好。Objective To investigate the clinical characteristics of neuroelectromyography(EMG)in 90 patients with type 2 diabetes mellitus(T2DM).Methods Ninety cases of T2DM patients in Liaocheng Second People’s Hospital from January 2019 to December 2019 were selected as the research objects.90 T2DM patients were divided into the group with peripheral nervous system diseases(28 cases)and the group without peripheral nervous system diseases(62 cases).All patients were examined by electromyography.The median nerve motor conduction velocity(MCV),common peroneal nerve MCV,median nerve sensory conduction velocity(SCV)and superficial peroneal nerve SCV were statistically analyzed.The receiver operating characteristic(ROC)curve was drawn and the area under the curve(AUC)was observed.The value of electromyography in the diagnosis of T2DM peripheral nervous system diseases was analyzed.Results MCV median nerve,MCV common peroneal nerve,SCV median nerve and SCV superficial peroneal nerve in patients with peripheral nervous system diseases were lower than those in patients without peripheral nervous system diseases(P<0.05).ROC curve was drawn,the results showed that MCV median nerve,MCV common peroneal nerve,SCV median nerve,SCV superficial peroneal nerve and combined detection in the diagnosis of peripheral nervous system diseases AUC=0.764,0.727,0.809,0.746,0.908.The combined detection had better evaluation value.Conclusions The EMG indexes of T2DM patients with peripheral nervous system diseases are significantly increased,and the diagnosis effect of EMG in peripheral nervous system diseases is better.
