沙库巴曲缬沙坦联合比索洛尔治疗扩张型心肌病的临床观察

摘　　要：目的观察扩张型心肌病(DCM)患者采用沙库巴曲缬沙坦与比索洛尔联合治疗的效果。方法抽取2018年1月至2019年6月于信阳市中心医院就诊的94例DCM患者,采用随机数字表法分为两组,每组47例。两组均采用常规治疗,对照组在常规治疗基础上采用比索洛尔治疗,观察组在常规治疗基础上采用比索洛尔与沙库巴曲缬沙坦联合治疗,连续治疗1个月观察治疗效果。治疗及治疗1个月,比较两组心功能、炎症因子水平,统计两组治疗期间不良反应。结果两组治疗1个月时左室射血分数(LVEF)水平较治疗前升高,左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)水平较治疗前降低,且观察组LVESD、LVEDD水平低于对照组,LVEF水平高于对照组(P<0.05);两组治疗1个月时白细胞介素-10(IL-10)水平较治疗前升高,肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)水平较治疗前降低,且观察组IL-10水平高于对照组,TNF-α、IL-6水平低于对照组(P<0.05);治疗期间,两组不良反应总发生率比较,差异未见统计学意义(P>0.05)。结论比索洛尔与沙库巴曲缬沙坦联合治疗可减轻DCM患者的炎症状态,有效改善心功能,且未增加不良反应,有一定安全性。Objective To observe the treatment effect of sacubitril valsartan combined with bisoprolol in the treatment of patients with dilated cardiomyopathy(DCM).Methods A total of 94 patients with dilated cardiomyopathy dmitted to Xinyang Central Hospital from January 2018 to June 2019 were selected prospectively,the random number table method was used to divide them into two groups,with 47 cases in each group.Both groups received routine treatment,the control group was treated with bisoprolol,the observation group was treated with sacubitril valsartan combined with bisoprolol,both were treated continuously for 1 month,and the therapeutic effect was observed.After 1 month of treatment,the cardiac function and inflammatory factor levels were compared between the two groups,and the adverse reactions during the treatment were counted.Results The level of left ventricular ejection fraction(LVEF)was increased and the levels of left ventricular end systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDD)were decreased in both groups after 1 month of treatment compared with those before treatment,and the LVESD and LVEDD levels of the observation group were lower than those of the control group,and the LVEF level of the observation group was higher than that of the control group(P<0.05);the level of interleukin-10(IL-10)in both groups was increased compared with that before treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)were decreased,and the level of IL-10 of the observation group was higher than that of the control group,the levels of TNF-αand IL-6 of the observation group were lower than those of the control group(P<0.05);during treatment,there was no statistical significant difference in the total incidence of adverse reactions of both groups(P>0.05).Conclusions Sacubitril valsartan combined with bisoprolol can reduce inflammatory reaction and efficiently improve cardiac function in the treatment of patients with DCM,and it will not increase the adverse reactions,with certain s
