超声引导注射聚桂醇泡沫硬化剂联合高位结扎术治疗大隐静脉曲张的临床疗效分析

摘　　要：目的探讨超声引导下注射聚桂醇泡沫硬化剂联合高位结扎术治疗大隐静脉曲张的临床疗效及应用价值。方法回顾性分析2018年1月至2019年12月于本院行手术治疗的120例大隐静脉曲张患者的临床资料,按照手术方式不同分为观察组与对照组,每组60例,观察组行超声引导下注射聚桂醇泡沫硬化剂联合高位结扎术,对照组行传统高位结扎剥脱术,比较两组住院时间、住院总费用、手术时间、术中出血量及术后并发症发生率。结果观察组住院时间明显短于对照组[(2.4±0.7)d vs.(4.7±0.7)d],住院总费用明显少于对照组[(5356.1±633.5)元vs.(7396.7±899.3)元],术中出血量少于对照组[(25.4±6.8)mL vs.(55.3±15.1)mL],差异有统计学意义(P<0.05)。观察组术后皮肤感觉障碍、皮下瘀血、切口感染发生率均低于对照组(P<0.05)。两组手术时间与术后肢体肿胀发生率比较差异无统计学意义。结论超声引导下注射聚桂醇泡沫硬化剂联合高位结扎术治疗大隐静脉曲张临床疗效确切,具有创伤小、恢复快及并发症少等优点,值得临床推广应用。Objective To investigate the clinical efficacy and application value of ultrasonic guided lauromacrogol foam sclerotherapy combined with high ligation of great saphenous vein for treatment of varicose vein.Methods The clinical data of 120 cases with varicose great saphenous vein in our hospital from January 2018 to December 2019 were retrospective analyzed.According to different surgical methods,they were divided into observation group and control group,with 60 cases in each group.The observation group received The observation group received ultrasonic guided lauromacrogol foam sclerotherapy combined with high ligation,and the control group received traditional high ligation and stripping.The hospital stay,total cost of hospitalization,operation time,intraoperative blood loss and postoperative complications were compared between two groups.Results The postoperative hospital stay in observation group was shorter than that in control group[(2.4±0.7)d vs.(4.7±0.7)d],the total cost of hospitalization[(5356.1±633.5)yuan vs.(7396.7±899.3)yuan],intraoperative blood loss[(25.4±6.8)mL vs.(55.3±15.1)mL],the difference was statistically significant(P<0.05).The incidence of postoperative skin sensory disturbance,subcutaneous blood stasis,and incision infection in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the operation time and postoperative limb swelling rate between the two groups.Conclusions Ultrasonic guided lauromacrogol foam sclerotherapy combined with high ligation of great saphenous vein for treatment of varicose vein is effective with advanteges of minimal trauma,quick recovery and fewer complications.
