周端运用膏方调治高血压病经验

摘　　要：周端教授认为,高血压病病位在头窍,与肝、肾、心、脾四脏密切相关,阴虚是其病理核心,贯穿疾病始终。高血压病总属阴虚阳亢,虚者多而实者少,可分为初期、中期、晚期,在高血压病初期、中期应用膏方较为适宜。周老师运用膏方调治高血压病时,注重辨证与辨病、辨体质相结合,注重情志调节。在开具膏方时,重视滋补肝肾、调气活血,以平肝熄风治其标,更注重补肝肾之阴治其本,常用熟地黄、黄精、枸杞子、山萸肉等补肾填精,滋水涵木,以阴制阳;重视脾胃功能,将培元顾胃法作为膏方治疗高血压病的重要治则,以白术、茯苓、薏苡仁、六神曲、山药等培元健脾;善加活血之品,治疗"血瘀"之标实之证;根据患者气之升降,酌加陈皮、香附、川楝子、枳壳等使其顺"苍天之气",使各脏"阴密阳固"。Professor Zhou Duan believes that hypertension is located in the head orifices,and closely related to the liver,kidney,heart and spleen.Yin deficiency is the core of its pathology and runs through the disease.Hypertension generally belongs to yin deficiency and yang hyperactivity with more deficiency than excess,which can be divided into early stage,middle stage and late stage.Paste formula is suitable for use at early stage,and middle stage.Professor Zhou pays attention to the combination of syndrome differentiation,disease differentiation and constitution differentiation,and emotional regulation when he uses paste formula to treat hypertension.When prescribing a paste formula,he emphasizes nourishing the liver and kidney,regulating qi and activating blood circulation,treating the symptoms by pacifying the liver and extinguishing wind,and treating the root cause by tonifying yin of the liver and kidney,so Shudihuang(Radix Rehmanniae Praeparata),Huangjing(Rhizoma Polygonati),Gouqizi(Fructus Lycii)and Shanyurou(Fructus Corni)are often used to nourish the kidney and supplement essence,nourish water and moisten wood,restrict yang with yin.He attaches great importance to the function of spleen and stomach,and takes method of banking up original qi and protecting the stomach as an important treatment principle in terms of hypertension,so he often uses Baizhu(Rhizoma Atractylodis Macrocephalae),Fuling(Poria),Yiyiren(Semen Coicis),Liushenqu(Massa Medicata Fermentata),Shanyao(Radix Paeoniae Alba seu Rubra),etc.to bank up original qi and strengthen the spleen.He is good at adopting medicinals for activating blood to treat excess syndrome such as blood stasis,etc.On the basis of the patient′s ascending and descending of qi,he properly adds Chenpi(Pericarpium Citri Reticulatae),Xiangfu(Rhizoma Cyperi),Chuanlianzi(Fructus Toosendan),Zhiqiao(Fructus Aurantii),etc.to follow their lucid qi to keep the balance between yin and yang.
