益心活血方联合抗凝疗法治疗非瓣膜性心房纤颤临床研究

摘　　要：目的:观察益心活血方联合抗凝疗法治疗非瓣膜性心房纤颤的临床疗效。方法:将102例非瓣膜性心房纤颤患者按照随机数字表法分为观察组和对照组,每组51例。两组患者均给予抗凝疗法,观察组另给予益气活血方治疗。观察两组患者治疗前后凝血功能、中医证候评分、栓塞事件和出血事件发生情况、治疗后临床疗效及不良反应情况。结果:两组治疗后凝血酶原时间、活化部分凝血活酶时间、凝血酶时间均升高,但观察组低于对照组,差异有统计学意义(P<0.05);观察组治疗后各项中医证候评分均低于对照组,差异有统计学意义(P<0.05);观察组缺血性脑卒中、心肌梗死、一般出血的发生率均低于对照组,差异有统计学意义(P<0.05);观察组有效率为92.16%,对照组有效率为74.51%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组消化不良、头痛、皮疹的发生率均低于对照组,差异有统计学意义(P<0.05)。结论:益心活血方联合抗凝疗法可改善非瓣膜性心房纤颤患者凝血功能和中医证候,降低缺血性脑卒中、心肌梗死、一般出血的发生率。Objective:To observe the clinical curative effect of Heart-Boosting and Blood-Activating Decoction combined with anticoagulant therapy on non valvular atrial fibrillation.Methods:A total of 102 patients with nonvalvular atrial fibrillation were randomly divided into the observation group and the control group,with 51 cases in each group.Both groups were first given anticoagulant therapy,and the observation group was additionally given Heart-Boosting and Blood-Activating Decoction.The coagulation function,TCM syndrome score,embolic events and bleeding events,clinical efficacy and adverse reactions were observed before and after treatment in the two groups.Results:After treatment,prothrombin time,activated partial thromboplastin time and thrombin time levels of the two groups were increased,but the above indexes of the observation group were lower than those of the control group,and all the differences were statistically significant(P<0.05).After treatment,the TCM syndrome scores of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05);The incidence of ischemic stroke,myocardial infarction and general bleeding in the observation group were lower than those in the control group,and all the differences were statistically significant(P<0.05).The effective rate of the observation group was 92.16%,which was higher than 74.51%of the control group,and the difference was statistically significant(P<0.05).The incidence of dyspepsia,headache and rash in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Heart-Boosting and Blood-Activating Decoction combined with anticoagulant therapy can improve the coagulation function and TCM syndromes of patients with nonvalvular atrial fibrillation,and reduce the incidence of ischemic stroke,myocardial infarction and general bleeding.
