加味玉屏风散用于反复呼吸道感染患儿(脾肺两虚型)的临床疗效评价

摘　　要：目的观察加味玉屏风散用于反复呼吸道感染患儿(脾肺两虚型)的临床疗效。方法将符合诊断标准的68例患儿随机分为治疗组和对照组各34例,对照组在患儿急性期给予西医常规对症治疗,缓解期不予处理;治疗组在患儿急性期处理同对照组,缓解期应用加味玉屏风散治疗,疗程2个月,治疗后1年随访。对比两组的临床疗效、病情变化、中医证候积分及血清免疫球蛋白水平。结果治疗组和对照组总有效率分别为90.62%、70.97%,治疗组总有效率高于对照组(P<0.05)。两组治疗均能减少患儿发病次数、缩短病程和改善病种,治疗组改善程度均优于对照组(P<0.05)。两组治疗均能改善患儿中医证候,与对照组相比,治疗组患儿的食少纳呆、形体消瘦、大便溏薄证候改善明显(P<0.05)。两组治疗均能提高患儿血清免疫球蛋白IgG、IgA、IgM水平,与对照组相比,治疗组效果更显著(P<0.05)。结论加味玉屏风散用于反复呼吸道感染患儿(脾肺两虚型)有较好的疗效,值得临床推广。OBJECTIVE To observe the clinical effect of modified Yupingfeng San for the treatment of children with lung-spleen QI-deficiency type of recurrent respiratory infection.METHODS 68 children with lung-spleen QI-deficiency type of recurrent respiratory infection were randomly divided into the treatment group and the control group,and both groups with 34 cases.During the acute phase,the patients of control group and treatment group were treated with conventional western medicine.And in the period of remission,the treatment group was treated with modified Yupingfeng San for two months and the patients were followed up for 1 year,but the control group was untreated.The clinical efficacy and pathogenetic changes of TCM syndrome scores and serum immunoglobulin levels were compared between the two groups.RESULTS The total effective rate of treatment group was significantly higher than that of the control group(90.62%vs.70.97%,P<0.05).Syndrome score,incidence of disease,course of the diseaseand time of disease indexes of the treatment group were better than those of the control group(P<0.05).The Chinese medicinal symptom scores including reduced appetite,emaciation and thin sloppy stool of the treatment group were significantly better than those of the control group(P<0.05).The levels of IgG、IgA、IgM of the treatment group were significantly higher than those of the control group(P<0.05).CONCLUSION Modified Yupingfeng San has good effect on children with lung-spleen QI-deficiency type recurrent respiratory infection.It is worthy of clinical promotion。
