关节灌洗联合自体富血小板血浆关节腔注射术治疗颞下颌关节骨关节炎的疗效评价

摘　　要：目的:评价用关节灌洗联合自体富血小板血浆关节腔注射术治疗颞下颌关节骨关节炎的临床疗效。方法:将柳州市人民医院接诊的60例颞下颌关节骨关节炎患者随机分为A组、B组和C组。为A组患者、B组患者和C组患者分别采用关节灌洗联合自体富血小板血浆关节腔注射术、关节灌洗联合透明质酸钠关节腔注射术、关节灌洗联合曲安奈德关节腔注射术进行治疗。然后对比三组患者的治疗效果。结果:1)治疗后6个月,A组患者和B组患者的非辅助最大开口度均大于C组患者,P<0.05;A组患者与B组患者的非辅助最大开口度相比,P>0.05。2)治疗后6个月,A组患者和B组患者咀嚼时颞下颌关节区疼痛感的评分均低于C组患者,P<0.05;B组患者咀嚼时颞下颌关节区疼痛感的评分低于A组患者,P<0.05。3)治疗后6个月,A组患者和B组患者颞下颌运动功能障碍的评分均低于C组患者,P<0.05;A组患者颞下颌运动功能障碍的评分低于B组患者,P<0.05。结论:采用关节灌洗联合自体富血小板血浆关节腔注射术治疗TMJOA的效果明显优于用关节灌洗联合曲安奈德关节腔注射术治疗TMJOA的效果,但并不优于用关节灌洗联合透明质酸钠关节腔注射术治疗TMJOA的效果。Objective:To evaluate the clinical effect of joint lavage combined with autologous platelet-rich plasma injection in the treatment of temporomandibular joint osteoarthritis.Methods:60 patients with TMJ osteoarthritis admitted to Liuzhou People’s Hospital were randomly divided into group A,group B and group C.Patients in group A,group B and group C were treated with joint lavage combined with autologous platelet rich plasma injection,joint lavage combined with sodium hyaluronate injection,and joint lavage combined with triamcinolone acetonide injection,respectively.Then the treatment effect of the three groups was compared.RESULTS:1)6 months after treatment,the unassisted maximum opening degree of group A and group B was higher than that of group C(P<0.05);The unassisted maximum opening degree of group A was compared with that of group B,P>0.05.2)Six months after treatment,the score of pain sensation of TMJ in group A and group B was lower than that in group C(P<0.05);The score of pain sensation of TMJ in group B was lower than that in group A(P<0.05).3)Six months after treatment,the scores of temporomandibular motor dysfunction in group A and group B were lower than those in group C,P<0.05;The score of temporomandibular motor dysfunction in group A was lower than that in group B(P<0.05).CONCLUSIONS:The therapeutic effect of joint lavage combined with autologous platelet-rich plasma injection in the treatment of TMJOA is obviously better than that of joint l avage combined with triamcinolone acetonide injection in the treatment of TMJOA,but not better than that of joint lavage combined with sodium hyaluronate injection in the treatment of TMJOA.
