尿微量白蛋白与血清胱抑素C联合应用于高尿酸血症并糖尿病肾损伤中的效果研究

摘　　要：目的探讨尿微量白蛋白(UMA)与血清胱抑素C(CysC)联合应用于高尿酸血症(HUA)并糖尿病肾损伤诊断中的效果。方法回顾性分析2019年6月-2020年5月该院收治的400例糖尿病肾损伤患者资料,其中197例肾功能损伤患者为研究组,203例无明确肾损伤患者为对照组。两组患者均行尿素氮(BUN)、血肌酐(SCr)、CysC与UMA检测,对比两组患者UMA、CysC、BUN、SCr水平,对比两组UMA、CysC、BUN、SCr与UMA联合CysC检测的阳性率。结果研究组UMA(94.6±20.3)mg/L、CysC(2.96±0.52)mg/L、BUN(10.22±2.32)mmol/L、SCr(160.65±25.66)μmol/L较对照组(39.8±10.6)mg/L、CysC(1.55±0.38)mg/L、BUN(4.85±1.65)mmol/L、SCr(77.65±13.55)μmol/L更高,差异有统计学意义(P<0.05)。在UMA、CysC、BUN、SCr与UMA联合CysC检测的阳性率对比中,研究组各项阳性率96.95%、97.97%、38.07%、44.67%、99.49%均较对照组25.62%、22.17%、5.91%、8.87%、50.25%更高,差异有统计学意义(P<0.05)。结论UMA与CysC联合应用于UMA并糖尿病肾损伤中效果确切,有效提高了疾病的诊断率,利于进一步开展诊疗工作,确保患者的就医质量。Objective To investigate the effect of combined application of urinary microalbumin(UMA)and serum cystatin C(CysC)in the diagnosis of hyperuricemia(HUA)and diabetic kidney injury.Methods A retrospective analysis of the data of 400 patients with diabetic admitted to the hospital from June 2019 to May 2020.Among them,197 patients with renal impairment were in the study group,and 203 patients with no clear renal injury were in the control group.Both groups of patients were tested for urea nitrogen(BUN),serum creatinine(SCr),CysC and UMA.The levels of UMA,CysC,BUN,SCr in the two groups were compared,and the two groups were compared with UMA,CysC,BUN,SCr and UMA combined with CysC testing.Results The study group UMA(94.6±20.3)mg/L,CysC(2.96±0.52)mg/L,BUN(10.22±2.32)mmol/L,SCr(160.65±25.66)μmol/L were compared with the control group(39.8±10.6)mg/L,CysC(1.55±0.38)mg/L,BUN(4.85±1.65)mmol/L,SCr(77.65±13.55)μmol/L were higher,and the difference was statistically significant(P<0.05).In the comparison of the positive rates of UMA,CysC,BUN,SCr and UMA combined with CysC,the positive rates of the study group were 96.95%,97.97%,38.07%,44.67%,and 99.49%compared with the control group by 25.62%,22.17%,5.91%,8.87%,and 50.25%were higher,and the difference was statistically significant(P<0.05).Conclusion The combined application of UMA and CysC in UMA and diabetic kidney injury has a definite effect,which effectively improves the diagnosis rate of the disease,facilitates further diagnosis and treatment,and ensures the quality of patients'medical treatment.
