广西分级诊疗制度实施过程中存在的问题与对策研究

摘　　要：分级诊疗制度是我国五项基本医疗卫生制度之首,有利于解决群众“看病难”和“看病贵”问题。广西壮族自治区出台了一系列政策并采取了积极措施,但科学有序的就医格局难以建立。该研究通过对分级诊疗制度在广西的实施现状进行分析,发现存在卫生资源配置结构失衡,三级甲等医院不断扩张,基层医疗卫生机构人才缺乏、全科医生严重不足,基层医疗机构与大医院之间双向转诊上下联动机制受阻,群众对基层医疗机构缺乏信任而习惯性地依赖城市大医院等问题。因此,政府应发挥主导作用,通过行政手段,明确医疗卫生机构布局和资源配置,全面提升基层医疗卫生机构服务能力,加强落实分级诊疗相关配套政策、完善上下联动机制,以柔性和刚性相结合的方式引导群众理性就医。The hierarchical diagnosis and treatment system is the first of the five basic medical and health systems in our country,which is conducive to solving the problems of"difficult and expensive medical treatment"for the masses.The Guangxi Zhuang Autonomous Region has issued a series of policies and adopted active measures,but it is difficult to establish a scientific and orderly medical treatment pattern.This study analyzed the current implementation of the hierarchical diagnosis and treatment system in Guangxi and found that there was an imbalance in the allocation of health resources,the continuous expansion of tertiary a hospitals,the shortage of talents in primary medical and health institutions,and the serious shortage of general practitioners.Primary medical institutions and large hospitals'two-way referral mechanism was hindered,and the masses lack trust in primary medical institutions and habitually rely on large urban hospitals.Therefore,the government should play a leading role,clarify the layout and resource allocation of medical and health institutions through administrative means,comprehensively improve the service capabilities of primary medical and health institutions,strengthen the implementation of supporting policies related to hierarchical diagnosis and treatment,improve the upper and lower linkage mechanism,and combine flexibility and rigidity ways to guide the masses to seek medical treatment rationally.
