解毒化癥汤灌肠联合常规西药治疗盆腔炎性包块湿热瘀结证临床研究

摘　　要：目的:观察解毒化癥汤灌肠联合常规西药对盆腔炎性包块湿热瘀结证患者血清T淋巴细胞亚群指标、超敏C-反应蛋白(hs-CRP)水平及中医证候的影响。方法:选取90例盆腔炎性包块湿热瘀结证患者,按随机数字表法分为试验组和对照组,每组45例。对照组给予注射用头孢曲松钠及奥硝唑注射液治疗,试验组在对照组基础上给予解毒化癥汤灌肠治疗。比较2组治疗前后血清T淋巴细胞亚群指标、hs-CRP水平及中医证候积分。结果:治疗后,2组血清hs-CRP水平均较治疗前降低(P<0.05),试验组血清hs-CRP水平低于对照组(P<0.05)。治疗后,2组血清CD3^(+)、CD4^(+)、CD8^(+)水平及CD4^(+)/CD8^(+)值均较治疗前改善(P<0.05);试验组血清CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)值均高于对照组(P<0.05),血清CD8^(+)水平低于对照组(P<0.05)。治疗后,2组神疲乏力、下腹痛、低热、腰骶部胀痛积分均较治疗前降低(P<0.05),试验组上述4项中医证候积分均低于对照组(P<0.05)。结论:解毒化癥汤灌肠有助于改善盆腔炎性包块湿热瘀结证患者血清T淋巴细胞亚群指标、hs-CRP水平,降低中医证候积分。Objective:To observe the effect of enema with Jiedu Huazheng tang combined with routine western medicine on levels of T-lymphocyte subsets and hypersensitivity C-reaction protein(hs-CRP)in serum as well as Chinese medicine syndrome of patients with pelvic inflammatory mass with dampness-heat accumulation syndrome.Methods:A total of 90 cases of patients with pelvic inflammatory mass with dampness-heat accumulation syndrome were selected,and divided into the experiment group and the control group according to the random number table method,45 cases in each group.The control group was given injection of ceftriaxone sodium and ornidazole injection,and the experiment group was additionally given enema with Jiedu Huazheng tang based on the treatment of the control group.Indexes of T-lymphocyte subsets and level of hs-CRP in serum,as well as Chinese medicine syndrome scores before and after treatment were compared between the two groups.Results:After treatment,levels of hs-CRP in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the level in serum in the experiment group was lower than that in the control group(P<0.05).After treatment,levels of CD3^(+),CD4^(+),and CD8^(+)in serum as well as CD4^(+)/CD8^(+)ratio in the two groups were improved when compared with those before treatment(P<0.05);levels of CD3^(+)and CD4^(+)in serum as well as CD4^(+)/CD8^(+)ratio in the experiment group were higher than those in the control group(P<0.05),and levels of CD8^(+)in serum were lower(P<0.05).After treatment,scores of lassitude of spirit and lack of strength,lower abdominal pain,low grade fever,and distending pain in the lumbosacral spine in the two groups were decreased when compared with those before treatment(P<0.05),and the four Chinese medicine syndrome scores above in the experiment group were lower than those in the control group(P<0.05).Conclusion:For patients with pelvic inflammatory mass with dampness-heat accumulation syndrom,enema with Jiedu Huazheng tang can conduce to improv
