闭合复位PFNA内固定治疗高龄老年女性股骨粗隆间骨折的疗效分析

摘　　要：目的分析在高龄老年女性股骨粗隆间骨折疾病治疗中闭合复位PFNA内固定治疗方法所取得的临床治疗效果。方法方便选取于2010年8月—2020年6月在苏州大学附属常熟市第一人民医院中进行股骨粗隆间骨折治疗的高龄老年女性患者共96例,随机数字分组法将患者分成两组,将抽到的奇数纳入到对照组,将抽到的偶数纳入到观察组,每组48例。对照组给予患者半髋关节置换术治疗方法,观察组给予患者闭合复位PFNA内固定治疗方法。观察两组患者的并发症发生率、临床指标及髋关节功能恢复优良率。结果并发症发生率,观察组为8.33%,低于对照组的27.08%,并发症发生率对比观察组低于对照组,差异有统计学意义(χ^(2)=5.790,P<0.05)。对照组手术时间为(84.36±15.47)min,骨折愈合时间为(21.43±4.76)周,术中出血量为(297.58±73.65)mL;观察组手术时间为(57.53±14.25)min,骨折愈合时间为(14.63±2.75)周,术中出血量为(137.35±31.24)mL;观察组手术时间、骨折愈合时间、术中出血量均低于对照组,差异有统计学意义(t=8.838、 8.570、 13.876,P<0.05)。髋关节功能恢复优良率,对照组为72.92%,观察组为91.67%,髋关节功能恢复优良率观察组高于对照组,差异有统计学意义(χ^(2)=5.790,P<0.05)。结论在高龄老年女性股骨粗隆间骨折疾病治疗中应用闭合复位PFNA内固定治疗方法,有助于降低患者的并发症发生率,改善患者的临床指标,提升患者的髋关节功能。Objective To analyze the clinical effects of closed reduction and PFNA internal fixation in the treatment of femoral intertrochanteric fractures in elderly women.Methods A total of 96 elderly female patients who were treated for femoral intertrochanteric fractures in the First People's Hospital of Changshu Affiliated to Soochow University from August 2010 to June 2020 were conveniently selected.The patients were divided into two groups by random number grouping.The odd numbers drawn were included in the control group,and the even numbers drawn were included in the observation group,with 48 cases in each group.The control group was given the treatment of hemi-hip arthroplasty,and the observation group was given the treatment of closed reduction and PFNA internal fixation.Observed the complication rate,clinical indicators and the rate of hip joint function recovery of the two groups of patients.Results The complication rate in the observation group was 8.33%,which was lower than 27.08%in the control group.The complication rate in the observation group was lower than that in the control group,and the difference was statistically significant(χ^(2)=5.790,P<0.05).The operation time of the control group was(84.36±15.47)min,the fracture healing time was(21.43±4.76)weeks,the intraoperative blood loss was(297.58±73.65)mL;the operation time of the observation group was(57.53±14.25)min,and the fracture healing time was(14.63±2.75)weeks,and the intraoperative blood loss was(137.35±31.24)mL;the operation time,fracture healing time,and intraoperative blood loss of the observation group were lower than those of the control group,and the difference was statistically significant(t=8.838,8.570,13.876,P<0.05).The excellent and good rate of hip joint function recovery was 72.92%in the control group and 91.67%in the observation group.The good and good rate of hip joint function recovery in the observation group was higher than that in the control group,and the difference was statistically significant(χ^(2)=5.790,P<0.05).Conclu
