摘 要:目的研究血清中中性粒细胞明胶酶相关脂质运载蛋白(neutrophil gelatinase-associated lipocalin,NGAL)、补体C1q、胱抑素C(cystatin C,Cys-C)、小而低密度脂蛋白胆固醇(small and low-density lipoprotein cholesterol,sd-LDL)、视黄醇(retinol blinding protein,RBP)、同型半胱氨酸(homocysteine,HCY)、尿蛋白(urinary protein,PRO)、尿视黄醇结合蛋白(urinary retinol blinding protein,U-RBP)、尿N-乙酰-β-D葡萄糖苷酶(urinary N-acetyl-β-D glucosidase,NAG)水平在肾移植术前、后及术后感染诊断中的临床价值。方法30例肾移植术前与20例健康体检者的NGAL、补体C1q、sd-LDL、Cys-C、HCY和RBP进行比较;检测各15例的肾移植术后感染与感染恢复中各指标水平;检测20例肾移植Cre异常和术后稳定的各指标水平;检测各18例的肾移植术前尿液、术后稳定尿液及健康查体者尿中NAG、PRO和RBP的水平;连续测定19例肾移植患者从术前、术后1d、3d、1周、2周、1月、2月、3个月后NGAL、补体C1q、sd-LDL、Cys-C、HCY和RBP水平并进行比较。结果肾移植术前组与对照组的NGAL、Cys-C、Hcy、BUN、Cre、RBP、补体C1q差异有统计学意义(P<0.05其中NGAL P<0.01);肾移植术后与术前NGAL、Cys-C、RBP、Cre的比较均差异有统计学意义(P<0.05);NGAL、Cys-C、RBP及Cre的术后1 d与术后3 d水平比较差异均有统计学意义(P<0.05),但1周~2个月间各指标两两比较差异均不具有统计学意义(P>0.05);肾移植术后感染与感染恢复患者中NGAL、Cys-C、Hcy、Cre、RBP水平差异有统计学意义(P<0.05其中NGAL、RBP P<0.01)。肾移植术后感染时NGAL、Cys-C的升高幅度及感染恢复时下降的幅度明显大于Cre(P<0.05);肾移植术后Cre异常与稳定患者的NGAL、Cys-C比较,差异有统计学意义(P<0.05其中NGAL P<0.01);术后1 d NGAL、Cys-C分别与Cre下降率比较差异有统计学意义(P<0.05),术后3 d NGAL、Cys-C分别与Cre阳性率比较差异有统计学意义(P<0.01);肾移植术前尿液�Objective To investigate clinical values of serum neutrophil gelatinase-associated lipocalin(NGAL),complement C1q,cystatin C(Cys-C),small and low-density lipoprotein cholesterol(sd-LDL),Retinol(RBP),homocysteine(HCY)and urinary protein(PRO),retinol blinding protein(RBP),and urinary N-acetyl-β-D glucosidase(NAG)in the diagnosis of infections before and after renal transplantation.Methods Thirty patients undergoing renal transplantation were compared for levels of NGAL,C1q,sd-LDL,Cys-C,HCY,and RBP with 20 healthy subjects.Each of the 15 renal transplant recipients and infection recovery groups were examined.Indices including 20 indicators of abnormal Cre transplantation and postoperative stable group were detected.18 cases of urine NAG,PRO and RBP in urine before renal transplantation,postoperative stable urine and healthy examination were measured.Levels of continuous measurement of 19 patients with renal transplantation from the preoperative,postoperative days 1,3,1 week,2 weeks,1 month,2 months,or 3 months after review were compared for NGAL,complement C1q,sd-LDL,Cys-C,HCY,and RBP.Results The differences of NGAL,Cys-C,RBP and Cre between patients of pre-operation and post-transplantation renal transplantation were significant(P<0.05).The levels of NGAL,Cys-C,RBP,and Cre on postoperative day 1 and postoperative day 3 were significantly different(P<0.05),while there was no significant difference between each week in each item between 1 week and 2 months(P>0.05).Before and after kidney transplantation,levels of NGAL,Cys-C,Hcy,BUN,Cre,RBP and complement C1q were significantly different(P<0.05 among which NGAL P<0.01).NGAL,Cys-C,Hcy,Cre,and RBP showed different levels between the infection group and infection recovery group after renal transplantation(P<0.05 for NGAL,RBP P<0.01).The increase of NGAL and Cys-C after renal transplantation infection was significantly greater than that of Cre(P<0.05).The difference between Cre abnormal group and stable group after renal transplantation showed that NGAL and Cys-C were si
