血浆D-二聚体、FDP、和尿ALB/Cr联合检测对早期糖尿病肾病临床诊断的价值

摘　　要：目的探究血浆D-二聚体、FDP和尿ALB/Cr联合检测对早期糖尿病肾病临床诊断的价值。方法选取我院2019年1月至2020年10月收治的113例早期糖尿病肾病患者作为EDN组,另选取50例单纯2型糖尿病患者作为T2DM组,并选取50例健康体检者作为健康对照组,检测三组受试者血浆D-二聚体、FDP的水平和尿ALB/Cr比值,并比较单个指标检测与联合检测对早期糖尿病肾病诊断的价值。结果EDN组血浆D-二聚体和FDP均高于T2DM组和健康对照组,尿ALB/Cr比值低于T2DM组和健康对照组,差异有统计学意义(P<0.05),血浆D-二聚体、FDP和尿ALB/Cr联合检测诊断早期糖尿病肾病的特异性与灵敏度分别为95.38%和96.21%,较单个指标检测高。结论血浆D-二聚体、FDP和尿ALB/Cr联合检测对早期糖尿病肾病的临床诊断具有一定的参考价值。Objective To explore the clinical diagnostic value of joint test of D-dimer,urinary FDP,and ALB/Cr for the early diabetic nephropathy.Methods We selected 100 early diabetic nephropathy patients,from January,2019 to October,2020 in our hospital,as the EDN group.Another 50 pure type 2 diabetic patients were selected as the group T2DM.In addition,50 healthy subjects were selected as the control group.D-dimer,FDP and urinary ALB/Cr were tested among the 3 groups.Then we compared the clinical diagnostic values between single index test and joint test for the early diabetes nephropathy.Results Group EDN had significantly higher D-dimer and FDP than group T2DM and the control group but lower urinary ALB/Cr than group T2DM and the control group(P<0.05).The specificity and sensitivity of the joint test for the diagnosis of the early diabetic nephropathy were 95.38%and 96.21%,respectively,which were both higher than single index test.Conclusion Joint test of D-dimer,FDP,and urinary ALB/Cr for the early diabetes and nephropathy is a valuable reference for the clinical practice.
