原发性干燥综合征患者脂代谢相关指标结果分析

摘　　要：目的研究原发性干燥综合征患者血脂相关指标水平的变化及与自身抗体的关系。方法选取原发性干燥综合征患者71例作为病例组,同期60例健康体检者作为对照组。比较两组血脂相关指标,包括总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、载脂蛋白a和载脂蛋白b。分析病例组血脂相关指标和自身抗体异常情况。另外,把病例组依据自身抗体分为SSA+SSB+,SSA+SSB-,SSA-SSB-三个亚组,比较血脂相关指标并分析血脂相关指标和自身抗体的关系。结果病例组的高密度脂蛋白胆固醇显著低于对照组,低密度脂蛋白胆固醇显著高于对照组,差异均有统计学意义(P<0.05)。病例组血脂异常者36例,占50.7%,并且以高胆固醇血症和低高密度脂蛋白胆固醇血症为主。病例组抗SSA抗体阳性者44例,占62.0%;抗SSA抗体和抗SSB均阳性者13例,占18.31%。此外,SSA+SSB+组的胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇与SSA-SSB-组相比显著降低,并且SSA+SSB+组的高密度脂蛋白胆固醇与SSA+SSB-组相比显著降低,差异均有统计学意义(P<0.05)。进一步相关分析发现抗SSA抗体、抗SSB抗体均与胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇呈显著负相关(P<0.05)。结论原发性干燥综合征患者的血脂水平存在一定比例的异常。在临床诊疗中应多加关注原发性干燥综合征患者特别是抗SSB抗体阳性患者的血脂水平,以降低心血管事件的发生风险。Objective To study the changes of blood lipid related indexes in patients with primary Sjogren’s syndrome and its relationship with autoantibodies.Methods 71 patients with primary Sjogren’s syndrome were selected as the case group,while 60 healthy subjects during the same period as the control group.The blood lipid related indexes of the two groups were compared,including total cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol,apolipoprotein a and apolipoprotein b.The indexes related to blood lipids and abnormal autoantibodies in the case group were analyzed.In addition,the case group was divided into three subgroups of SSA+SSB+,SSA+SSB-,and SSA-SSB-according to autoantibodies.The blood lipid related indexes were compared and the relationship between blood lipid related indexes and autoantibodies was analyzed.Results The high-density lipoprotein cholesterol in the case group was significantly lower than that in the control group,while the low-density lipoprotein cholesterol in the case group was significantly higher than that in the control group(P<0.05).There were 36 cases of abnormal blood lipids in the case group,accounting for 50.7%,which were mainly hypercholesterolemia and low high-density lipoprotein cholesterol.In the case group,44 cases(62.0%)were anti-SSA positive,and 13 cases(18.31%)were anti-SSA positive and anti-SSB positive.In addition,the levels of cholesterol,high density lipoprotein cholesterol and low-density lipoprotein cholesterol in the SSA+SSB+group were significantly lower than those in the SSA-SSB-group,and the high density lipoprotein cholesterol in the SSA+SSB+group was significantly lower than that in the SSA+SSB-group.Furthermore,the correlation analysis showed that anti-SSA antibody and anti-SSB antibody were negatively correlated with cholesterol,high density lipoprotein cholesterol and low-density lipoprotein cholesterol.Conclusion There is a certain proportion of abnormal blood lipid level in patients with primary Sjogren’s synd
