化学发光免疫法测定人体甲状腺激素的性能验证及其临床应用

摘　　要：目的评估化学发光免疫系统在临床甲状腺素检测中的应用效果。方法选取我院2018年6月至2019年11月区间内的100例甲亢患者、100例甲低患者和100例甲状腺功能正常者作为试验对象,将化学发光免疫法(CLIA)与酶联免疫吸附法(ELISA)、放射免疫测定法(RAI)检测结果比较。结果使用化学发光免疫系统实测血清促甲状腺素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)时,所得线性范围、准确度、总精密度均较好,标准变异系数(CV)1.58%~6.44%、灵敏度较高且方法总回收率在99.7%~104.6%;且实际样本检测结果优于ELISA法、RAI法。结论利用化学发光免疫法对甲状腺功能异常患者进行血清中甲状腺素及其抗体检测,可显著提高检测结果准确性并缩短检测周期,具有较高的临床医学推广价值。Objective To evaluate the application effect of chemiluminescence immune system in clinical thyroid hormone detection.Methods 100 patients with hyperthyroidism,100 patients with hypothyroidism and 100 patients with normal thyroid function in our hospital from June,2018 to November,2019 were selected as the test objects.The detection results of chemiluminescence immunoassay(CLIA),enzyme-linked immunosorbent assay(ELISA)and radioimmunoassay(RAI)were then compared.Results When the serum TSH,FT3 and FT4 were measured by chemiluminescence immune system,the linear range,accuracy,total precision,and standard coefficient of variation(CV)were 1.58%-6.44%,and the sensitivity was high and the total recovery rate was 99.7%-104.6%.The detection results were better than those of ELISA and RAI methods.Conclusion Chemiluminescence immunoassay can significantly improve the accuracy of detection results and shorten the detection cycle in patients with thyroid dysfunction,which has a high clinical value.
