中医综合护理措施联合常规疗法对股骨颈骨折术后早期康复效果的影响  

Effect of Comprehensive Nursing Measures of Chinese Medicine Combined with Routine Therapy on Early Rehabilitation After Surgery of Femoral Neck Fractures

摘　　要：目的:观察在现代医学常规治疗和护理的基础上加予由艾灸、耳穴压豆和中医饮食指导组成的中医综合护理措施干预股骨颈骨折术后患者的早期康复效果。方法:将88例符合要求的患者随机分为2组各44例。2组均行空心螺钉内固定术,术后给予抗感染治疗和预防深静脉血栓形成,并实施认知教育、心理护理、防治并发症和康复锻炼等综合护理措施,观察组加予由艾灸、耳穴压豆和中医饮食指导组成的中医综合护理措施。2组疗程均为4周。于术后1 d、术后2周和术后4周评价髋关节功能(Harris评分),统计术后并发症发生率,患者于出院当天评价护理满意度。结果:术后4周内,2组Harris评分均逐渐升高(P<0.01)。观察组术后2周和术后4周的Harris评分均高于对照组(P<0.01)。术后4周,观察组髋关节功能情况优于对照组(P<0.05)。观察组并发症累积发生率为34.09%,低于对照组的61.36%,差异有统计学意义(P<0.05)。观察组护理满意度高于对照组(P<0.05)。结论:在现代医学常规治疗和护理的基础上加予由艾灸、耳穴压豆和中医饮食指导组成的中医综合护理措施干预股骨颈骨折术后患者,可有效改善患者的髋关节功能,减少术后并发症发生,提高患者对护理工作的满意度。Objective:To observe the intervention effect of comprehensive nursing measures of Chinese medicine constituted by moxibustion,auricular point pressing with beans,and dietary guidance of Chinese medicine based on routine treatment and nursing of modern medicine on early rehabilitation of patients after surgery of femoral neck fractures.Methods:A total of 88 cases of patients who met requirements were randomly divided into two groups,44 cases in each group.Both groups were given internal fixation with hollow screws.After surgery,they were given anti-infection treatment,prevention of deep venous thrombosis,as well as cognitive education,mental nursing,prevention and treatment of complications,rehabilitation exercise,and other comprehensive nursing measures.The observation group was additionally given comprehensive nursing measures of Chinese medicine constituted by moxibustion,auricular point pressing with beans,and dietary guidance of Chinese medicine.Both groups were treated for four weeks.One day,two weeks,and four weeks after surgery,the hip joint function was evaluated by the Harris score,and the incidence of postoperative complications was counted.Nursing satisfaction was evaluated by patients when discharge.Results:During the four weeks after surgery,the Harris scores in the two groups were gradually increased(P<0.01).The Harris scores two weeks and four weeks after surgery in the observation group were higher than those in the control group(P<0.01).Four weeks after surgery,the hip joint function in the observation group was better than that in the control group(P<0.05).The cumulative incidence of complications was 34.09% in the observation group,lower than that of 61.36% in the control group,the difference being significant(P<0.05).Nursing satisfaction in the observation group was higher than that in the control group(P<0.05).Conclusion:For patients after surgery of femoral neck fractures,the intervention of comprehensive nursing measures of Chinese medicine constituted by moxibustion,auricular point pressin
