摘 要:目的通过构建甲状腺结节疾病关联网络,揭示其相关疾病的关联关系。方法自山西省某三甲医院电子病案系统检索2008年8月1日-2017年8月31日出院诊断包含甲状腺结节的病例,利用共词分析方法,构建甲状腺结节疾病共现网络,并使用Gephi软件做可视化展示。结果 2857名患者中女性1818人、男性1039人,男女比为1:1.75;年龄集中于40岁及以上,占比达95.27%;住院患者人次逐年递增,于2012年起迅速增加,入住科室以内分泌科、心内科、老年病科、普外科为主;经相关性分析,伴随甲状腺结节出现的诊断主要为高血压、脂肪肝、脑梗死等;共现网络聚类归总为8个社区,可总结为7大集群。结论甲状腺能够通过多种直接或间接途径对其他器官产生影响,因此甲状腺结节与多种疾病诊断具有共现性;甲状腺结节与高血压、脂肪肝、脑梗死、高脂血症及2型糖尿病等多种疾病具有一定的关联性,为甲状腺结节综合诊疗提供新的参考。Objective To construct the thyroid nodule disease association network and reveal the association of related diseases. Methods From an electronic medical record system in a third-grade a hospital in Shanxi Province, the cases containing thyroid nodules were retrieved from discharge diagnosis from August 1, 2008 to August 31, 2017. The co-occurrence network of thyroid nodules was constructed by using the co-word analysis method, and the visual display was performed by using Gephi software. Results Among the 2857 patients, 1,818 were females and 1,039 were males, with a male-to-female ratio of 1:1.75. Aged 40 and above, accounting for 95.27%;The number of inpatients has been increasing year by year, and has been increasing rapidly since 2012. The department of endocrinology, Cardiology, Geriatrics and general surgery are the main departments.According to correlation analysis, the diagnosis with thyroid nodules was mainly hypertension, fatty liver, cerebral infarction, etc. The cluster of co-occurrence network is divided into 8 communities, which can be summarized into 7 clusters. Conclusion The thyroid gland can affect other organs through a variety of direct or indirect ways, so thyroid nodule has co-occurrence with the diagnosis of a variety of diseases. Thyroid nodules are associated with hypertension, fatty liver, cerebral infarction, hyperlipidemia, type 2 diabetes and other diseases, providing a new reference for the comprehensive diagnosis and treatment of thyroid nod ules.
