湖南省某三甲妇幼医院628例超/极低出生体重儿临床分析  

Analysis on 628 Cases of Extremely/Very Low Birth Weight Infants in a Three-A Women and Children’s Hospital of Hunan Province

摘　　要：目的探讨超/极低出生体重儿的临床特点,为超/极低出生体重儿的治疗和预防提供参考。方法某三甲妇幼医院调取2016年1月1日-2019年12月31日期间628例超/极低出生体重儿住院病案信息资料,统计分析孕母情况、患儿并发症、平均住院日、转归、住院费用等情况,计算超/极低出生体重儿的出生体重与胎龄、孕母平均年龄、平均住院天数、平均住院总费用的相关性。结果超/极低出生体重儿孕母主要有妊娠合并贫血44.55%、多胎妊娠44.18%、胎儿生长受限42.33%和未足月胎膜早破41.77%等状况;超/极低出生体重儿并发症的顺位为新生儿肺炎、贫血、低蛋白血症、败血症、代谢性酸中毒、呼吸窘迫综合征和呼吸性酸中毒等;超/极低出生体重儿平均住院日约为39.76天、平均住院费用约为70548.71元,超/极低出生体重儿的出生体重与胎龄呈正相关,与孕母平均年龄、平均住院天数、平均住院总费用呈负相关。结论加强高危孕妇围产期监管,做到早发现、早治疗和早期干预,对减少超/极低出生体重儿的出生率和提高出生儿生存质量有重要意义。Objective To analysis the clinical characteristics of Extremely/Very low birth weight infants(ELBWI/VLBWI), and provide references for the treatment and prevention of ELBWI/VLBWI. Methods Hospitalization medical records for 628 cases of ELBWI/VLBWI from January 1, 2016 to December 31, 2019 were collected and analyzed by software such as Excel. Results ELBWI/VLBWI maternal medical records mainly included pregnancy complicated with anemia(44.55%), multiple pregnancy(44.18%), fetal growth restriction(42.33%) and preterm premature rupture of membranes(41.77%);The order of ELBWI/VLBWI complications was neonatal pneumonia, anemia, hypoalbuminemia, sepsis, metabolic acidosis, respiratory distress syndrome, and respiratory acidosis. The average hospital stay of ELBWI/VLBWI was about 39.76 days, and the average hospitalization cost was about 70548.71 yuan. The birth weight of ELBWI/VLBWI was positively correlated with gestational age, and was negatively correlated with the average age of pregnant mothers, the average length of hospitalization and total hospitalization expenses. Conclusions Strengthening the perinatal supervision of high-risk pregnant women, and preventing the happening of premature birth should have important clinical significance for reducing their birth rate of ELBWI/VLBWI, and can improve the quality of life of children.
