摘　　要：目的探讨剖宫产患者住院费用的主要影响因素,为减轻患者疾病经济负担、合理控制住院费用过速增长提供参考。方法选择江苏省某三级医院2017年1月1日-2019年12月31日4040例剖宫产患者进行通径分析住院费用的直接和间接影响因素。结果住院天数对住院费用的总效应为0.617,是主要影响因素,是否输血对住院费用的总效应为0.271,是次要影响因素,其他因素包括疾病诊疗信息、出院年份、患者年龄、药占比、支付方式等,可通过住院天数间接影响住院费用。结论提高诊疗水平、加强医疗质量管理缩短无效住院天数、优化住院费用结构、降低药品费用占比、加强孕产期宣教及干预等一系列综合措施加强管控,有效缓解住院费用的过速增长。Objectives To explore the major influencing factors of hospitalization expenses of cesarean delivery patients,and to provide reference for reducing the economic burden of disease and controlling the excessive growth of hospitalization expenses.Methods To select 4040 cesarean delivery patients in a tertiary hospital of Jiangsu province from January 1st 2017 to Decermber 31st,2019 for path analysis to research the direct and indirect influencing factors of hospitalization expenses.Results The total effect of hospitalization days on hospitalization expenses was 0.617,which was the main influencing factor.The total effect of blood transfusion on hospitalization expenses was 0.271,which was the second influencing factor.Other factors,including disease diagnosis and treatment information,discharge year,patient age,drug proportion and payment method,indirectly affected the hospitalization expenses through hospitalization days.Conclusions A series of comprehensive measures could be taken to improve the level of diagnosis and treatment,strengthen medical quality management,shorten the invalid length of stay,optimize the structure of hospitalization expenses,reduce the proportion of drug expenses,and strengthen the education and intervention during pregnancy and childbirth,so as to effectively alleviate the rapid growth of hospitalization expenses.
