输尿管镜尿道会师术与开放尿道吻合术治疗早期前尿道损伤的疗效比较

摘　　要：目的:比较输尿管镜尿道会师术与开放尿道吻合术治疗早期男性前尿道损伤的临床疗效与安全性。方法:回顾性分析2010年8月~2018年12月间深圳市南山区蛇口人民医院收治的86例前尿道损伤患者的临床资料,根据手术方式分为内镜组56例和开放组30例,比较两组手术时间、术中出血量、住院时间以及术后并发症发生情况。结果:内镜组患者手术时间为(16.15±5.40)min,明显少于开放组的(75.20±12.81)min,术中出血量为(35.25±11.36)ml,明显少于开放组的(115.6±33.65)ml,住院时间为显著低于开放组的(14.28±3.10)d和(20.16±4.79)d,差异有统计学意义(P<0.01)。结论:输尿管镜尿道会师术治疗前尿道损伤具有创伤小、出血少,手术时间短、并发症少等优点,在临床治疗早期男性前尿道损伤时可作为优先选择方案。Objective To compare the clinical efficacy and safety of ureteroscopic urethral realignment and open urethral anastomosis in the treatment of early anterior urethral injury.Method The clinical data of 86 patients with anterior urethral injury admitted to Shenzhen Skekou people's Hospital from August 2010 to December 2018 were retrospectively analyzed.According to the way of operation,they were divided into endoscopic group 56 cases and open group 30 cases.The operation time,blood loss volume during operation,postoperative hospital complications were compared between the two groups.Results The operation time of the endoscopic group was(16.15±5.40)min,which was significantly less than(75.20±12.81)min in the open group.The blood loss volume in the endoscopic group was(35.25±11.36)ml,which was significantly less than(115.6±33.65)ml in the open group.The hospital stay in the endoscopic group was(14.28±3.10)days,which was significantly less than(20.16±4.79)days in the open group.All differences were statistically significantly(P<0.01).Conclusion Ureteroscopic urethral realignment has the advantages of less trauma,less bleeding during operation,shorter operation time and less complications after operation,so it can be used as a priority option in the clinical treatment of early male anterior urethral injury.
