摘 要:目的评价经电子支气管镜冷冻联合氩等离子体凝固(APC)方法介入治疗恶性气道狭窄的疗效与安全性。方法2016年2月~2018年12月,对30例恶性气道狭窄的患者进行冷冻联合APC治疗,分析患者临床资料、并发症及术后转归等情况。结果(1)经气管镜使用冷冻联合APC方法治疗恶性气道狭窄30例,总有效率达100%,KPS评分由术前的(63.57±5.24)分增加至术后(79.25±7.62)分(P<0.05);(2)腔内介入治疗术中最多见的并发症有出血、一过性低氧血症、心律失常,均能得到有效控制而不影响操作。结论经气管镜下冷冻联合APC介入治疗恶性气道狭窄,可使患者迅速缓解气道梗阻症状,为肿瘤后续治疗创造有利条件。Objective To explore the safety and the efficacy of bronchoscopic argon plasma coagulation(APC)combined with CO2 freezing for the treatment of airway granulation hyperpiasis after pediatric airway stenting.Methods From February 2016 to December 2018,APC combined with CO2 cryotherapy comlications and postoperative outcomes and follow-up date were analyzed.Results(1)Via endoscopic interventional by cryoablation and hot ablat ion treatments with malignant airway stenoses 30,the total effective rate arrived to 100%,KPS score was arised form 63.57±5.24 to 79.25±7.62(P<0.05)with statistical signif icance;(2)There were no serious complications occured perioperation except hyoxmia,bledding and arrhythima,but most of which could be controled.Conclusion The treatment of malignant airway stricture under tracheal microscope combined with APC intervention can quickly relieve the symptoms of airway obstruction and create favorable conditions for the follow-up treatment of the tumor.
